Het is een goede week in de zoektocht naar een coronavaccin, met positief nieuws uit de klinische onderzoeken van AstraZeneca & Oxford, Pfizer & BioNTech en CanSino. Ook Johnson & Johnson zet de eerste stappen richting de kliniek, met een door Janssen in Leiden ontwikkeld vaccin. Bovendien krijgt dit vaccin nog een extra Nederlands tintje…

Janssen haalt trials naar Nederland

Johnson & Johnson heeft een mooie opsteker voor Nederland: na België en de VS staan ook wij op de rol voor trials! Een prachtige resultaat voor iedereen die zich de afgelopen tijd heeft ingezet om de vergunningaanvraag voor klinisch onderzoek met cel- en gentherapie in Nederland te versnellen en te verbeteren.

Nederlandse bijdrage tegen corona in de spotlight

De multinationals zijn niet de enigen die zich wagen in de strijd tegen het coronavirus. Op de besloten website van het Financieele Dagblad schuilt een mooie tweeluik over de bijdrage van Nederlandse partijen in de ontwikkeling van vaccins en geneesmiddelen. De vaccins komen aan bod in deel 1, waarbij de bijdragen van Intravacc, Halix, Mymetics, Janssen en Viroclinics toegelicht worden. In deel 2 ligt de focus op geneesmiddelen en staan ISA Pharmaceuticals, Protinhi Therapeutics en Pharming in de schijnwerpers.

Met zoveel inspanningen zowel binnen als buiten Nederland is de vraag niet of, maar wanneer biotech deze crisis weet te bezweren. We zien namelijk steeds vaker dat biotech de onmisbare schakel vormt voor oplossingen van vele maatschappelijke vraagstukken. De huidige situatie laat bovendien duidelijk zien dat we gebaat zijn bij een snelle en vooral ook betere route van lab naar praktijk. Daarom zijn we bij HollandBIO erg benieuwd welke lessen beklijven, zodat we de bijdrage van biotech aan onze samenleving duurzaam kunnen verbeteren!

Door coronapandemie uitgestelde tentoonstelling over uitbraken van besmettelijke ziekten alsnog door Koning Willem-Alexander geopend

Koning Willem-Alexander heeft de tentoonstelling ‘Besmet!’ op donderdag 16 juli in Rijksmuseum Boerhaave in Leiden geopend. De opening zou al op 15 april plaatsvinden, maar werd geannuleerd vanwege de uitbraak van de coronapandemie. De tentoonstelling gaat over de uitbraken van besmettelijke ziekten en hoe die het leven kunnen ontwrichten. Ook sluit de tentoonstelling aan bij de recente uitbraak van het coronavirus en bij de maatschappelijke discussie over de vaccinatiegraad.

Wat ons betreft laat de tentoonstelling goed zien wat het onverminderd grote belang is van vaccinaties en bovenal tipt HollandBIO deze tentoonstelling als een leerzaam zomeruitje! Meer weten? Lees verder bij Rijksmuseum Boerhaave.

Demonstrating the Value of Drugs for Rare Diseases – 8 Common Challenges and How to Address Them Before They Arise

No matter how robust in design, clinical trials can never fully address the uncertainty surrounding the comparative effectiveness and safety of a novel product in the real world. This is particularly pertinent in rare diseases for several reasons: small sample sizes and heterogeneous populations create challenges in demonstrating clinically significant benefits, randomized head-to-head comparisons are often unfeasible or unethical, and relationships between measurable trial endpoints and patient-relevant outcomes are under-studied and poorly understood. When preparing for health technology assessments and reimbursement discussions for a rare disease product, we believe that it is both feasible and advisable to anticipate and subsequently minimize the key uncertainties before starting the later phases of clinical programmes. While this is not a new proposition, our experience suggests that this rarely happens in practice, and there is therefore a need to reinforce the value of this simple recommendation.

In this White Paper, we outline eight key challenges that manufacturers face when demonstrating the value of rare disease products to decision-makers, and set out our framework for proactively overcoming these challenges. Using this framework, we believe that manufacturers can achieve earlier access to market, at a price that reflects product value.

Door de coronacrisis biedt dit initiatief van de Europese Commissie namelijk nieuwe kansen om het farmadebat in Nederland naar een ‘next level’ te brengen: hoe zorgen we samen – met een florerende Europese farmasector – dat nieuwe medicijnen voor patiënten sneller en overal beschikbaar zijn én betaalbaar blijven. New AMR Action Fund steps in to save collapsing antibiotic pipeline with pharmaceutical industry investment of US$1 billion

Last week, more than 20 leading biopharmaceutical companies announced the launch of the AMR Action Fund, a ground-breaking partnership that aims to bring 2-4 new antibiotics to patients by 2030. These treatments are urgently needed to address the rapid rise of antibiotic-resistant infections –also called antimicrobial resistance, or AMR. The companies have raised so far nearly US$1 billion new funding to support clinical research of innovative new antibiotics that are addressing the most resistant bacteria and life-threatening infections. Through the AMR Action Fund, pharmaceutical companies will join forces with philanthropies, development banks, and multilateral organizations to strengthen and accelerate antibiotic development.The Fund will focus on urgent public health needs. It will provide much needed financial resources, as well as important technical support to help biotech companies bring novel antibiotics to patients.

The AMR Action Fund, an initiative of the international body representing the R&D pharmaceutical industry (International Federation of Pharmaceutical Manufacturers & Associations, IFPMA), was announced at simultaneous virtual launch events in Berlin, Germany, and Washington, D.C., USA, with a third event in Tokyo, Japan taking place on July 10.

AMR is a looming global crisis that has the potential to dwarf COVID-19 in terms of deaths and economic costs. While tragically the death toll of COVID-19 continues to rise, each year 700,000 people are dying from AMR. In some of the most alarming scenarios, it is estimated that by 2050 AMR could claim as many as 10 million lives per year.

“Unlike COVID-19, AMR is a predictable and preventable crisis. We must act together to rebuild the pipeline and ensure that the most promising and innovative antibiotics make it from the lab to patients,”said Thomas Cueni,Director General of the IFPMA, one of the organizers of the new fund. He adds: “The AMR Action Fund is one of the largest and most ambitious collaborative initiatives ever undertaken by the pharmaceutical industry to respond to a global public health threat”.

The world urgently needs new antibiotics, but there are few in the pipeline because of a paradox: despite the huge societal costs of AMR, there is currently no viable market for new antibiotics. New antibiotics are used sparingly to preserve effectiveness, so in recent years, a number of antibiotic-focused biotechs have declared bankruptcy or exited this spacedueto the lack of commercial sustainability, resulting in the loss of valuable expertise and resources. The consequenceis a huge public healthneed for new antibiotics, but a lack of funding available for antibiotic R&D, particularly the later stages of clinical research. This creates a “valley of death” between discovery and patient access.

“With the AMR Action Fund, the pharmaceutical industry is investing nearly US$1 billion to sustain an antibiotic pipeline that is on the verge of collapse, a potentially devastating situation that could affect millions of people around the world,” said David Ricks, Chairman and CEO of Eli Lilly and Company and President of IFPMA. “The AMR Action Fund will support innovative antibiotic candidates through the most challenging later stages of drug development, ultimately providing governments time to make the necessary policy reforms to enable a sustainable antibiotic pipeline.”

While the AMR Action Fund is an important step in addressing the challengeof AMR, policymakers across the globe must enact market-based reforms, including reimbursement reform and new pull incentives, to revitalize the antibiotics market and drive sustainable investments in antibiotic R&D. Until then, the biopharmaceutical industry is taking action now to support the current pipeline of antibiotics.

With this investment from leading biopharmaceutical companies, the AMR Action Fund will be the largest collective venture ever created to address AMR. The AMR Action Fund will:

  • Invest in smaller biotech companies focused on developing innovative antibacterial treatments that address the highest priority public health needs, make a significant difference in clinical practice, and save lives.
  • Provide technical support to portfolio companies, giving them access to the deep expertise and resources of large biopharmaceutical companies, to strengthen antibiotic development, and support access and appropriate use of antibiotics.
  • Bring together a broad alliance of industry and non-industry stakeholders, including philanthropies, development banks, and multilateral organizations, and help encourage governments to create market conditions that enable sustainable investment in the antibiotic pipeline.

The AMR Action Fund expects to invest more than US$1 billion with the support of future partners into a portfolio of companies to address the funding gap for the financing of antibiotic development. The Fund is expected to be operational during the fourth quarter of 2020.

Slimmer meten wint aan draagvlak

De Koninklijke Nederlandse Akademie van Wetenschappen (KNAW) gaat onderzoeken hoe veelbelovende ontwikkelingen in wetenschap en technologie kunnen bijdragen aan efficiëntere ontwikkeling van geneesmiddelen en maatwerk in de klinische praktijk. HollandBIO juicht dit onderzoek toe, omdat het naadloos aansluit bij ons Slimmer Meten project.

De toenemende zorgvraag zet onze gezondheidszorg onder druk. Tel daarbij op dat het ontwikkelen van nieuwe geneesmiddelen lang duurt, kostbaar en risicovol is. Wanneer er uiteindelijk een geregistreerd en vergoed geneesmiddel beschikbaar is, is het nog maar de vraag of het middel ook werkt bij een specifieke patiënt. Dat kan sneller en beter.

Door in Nederland werk te maken van slimme zorg die leidt tot de juiste én beste behandeling voor de juiste patiënt op het juiste moment, komt maatwerk en personalised medicine in de zorg een stap dichterbij.

Gelukkig worden er wereldwijd én in onze achterban al vele innovatieve technologieën ontwikkeld en soms ook al toegepast. Denk aan modellen en testen gebaseerd op (stam-) cellen, biomarkers (DNA, RNA, eiwit, metabolieten), weefsels of computermodellen. Voorbeelden zijn: organoïden, organs-on-a-chip, de toxtracker, de MAT-test, de AMS microtracer of zelfs innovatieve productiemethoden, zoals het gebruik van mobiele RNA-molecuul printers.

De inzet van dit soort innovatieve technologieën en toepassingen biedt volop kansen bij het ontwikkelen van (high-throughput) screening systemen, als alternatief voor dierproeven, voor veiligheidstesten en voor diagnostische toepassingen (bv. companion diagnostics). Daarmee kunnen ze bijdragen aan snellere en betere ontwikkeling én inzet van nieuwe geneesmiddelen.

Ondanks het wenkend perspectief van personalised medicine blijkt de route van lab naar praktijk voor deze technologische innovaties hobbelig. Recent inventariseerden wij binnen ons Slimmer Meten project de uitdagingen op weg van lab naar praktijk. Om wat voorbeelden te noemen:

  • Gebrek aan financiering voor ontwikkeling, kwalificatie, validatie en commercialisatie;
  • In tegenstelling tot bij geneesmiddelen is er geen duidelijke route voor registratie en vergoeding voor deze innovaties en toepassingen;
  • Regelgeving tussen verschillende Europese landen loopt uiteen;
  • Onduidelijkheid en terughoudendheid bij verschillende stakeholders over wanneer en hoe een technologie of test, een dierpoef of fase I/II klinische trial kan vervangen;
  • Uiteenlopende belangen en gebrek aan de juiste kennis, procedures, processen en infrastructuur belemmeren toepassing in de klinische praktijk.

Werk aan de winkel dus! Daarom maakt HollandBIO zich hard voor een innovatieklimaat waarin biotechondernemers en bedrijven kunnen excelleren. Intravacc to in-license CimCure’s iBoost technology, expanding its vaccine platform portfolio

Intravacc, a worldwide leading translational research and development vaccine organization with an extensive track record in developing viral and bacterial vaccines, signed a strategic partnership agreement with Dutch based CimCure, focusing on cancer immunotherapies for solid tumors, for in-licensing CimCure’s iBoost technology. This agreement offers Intravacc access to this technology in developing new vaccines and for providing services involving the in-licensed technique in mutually agreed countries. This technology also enables Intravacc to expand its translational expertise towards the development of cancer vaccines.

The iBoost (immune boost) technology was developed at Amsterdam UMC, lab-location VUMC, by Prof. Dr. Arjan Griffioen, professor of experimental oncology and CSO at CimCure. This technology overcomes limitations over current methods for inducing antibody responses against cancer specific antigens. iBoost is currently applied to elicit antibody responses against the tumor vasculature, a strategy that conquers the problem of drug resistance. This treatment strategy has a long-term efficacy and is extremely cost-effective. Proof-of-concept has been shown in several preclinical models.

By using the patented iBoost technology, Intravacc and CimCure are also involved in the joint development of a COVID-19 vaccine. CimCure/Amsterdam UMC received a 1.3 million euro grant from Health Holland, an investment organization of the Ministry of Economic Affairs. In the collaboration, CimCure will focus on the design and preclinical validation of three different types of COVID-19 vaccine concepts, while Intravacc will be responsible for the vaccine process development, the pilot production of the candidate vaccine under GMP conditions and for a phase I clinical trial.

ISA Pharmaceuticals receives US Orphan-Drug Designation for ISA101b in HPV16-positive Cervical Cancer

ISA Pharmaceuticals B.V., a clinical-stage company dedicated to developing rationally designed immunotherapeutics for oncology and infectious disease, today announced that it received Orphan Drug Designation from the Food and Drug Administration (FDA) in the USA for its lead product ISA101b for treatment of Human Papilloma Virus type 16 (HPV16)-positive cervical cancer.

According to the latest statistics of the Centers for Disease Control and Prevention, HPV causes more than 34,000 cases of cancer in the United States each year. Cervical cancer is the most common HPV-associated cancer in women, with HPV16 being responsible for approximately 50% of cases. HPV16-positive cervical cancer is the number 2 cause of cancer for women in the age 15-44 in the US. At 1% annually, rates of cervical cancer have been dropping only marginally due to prevention through screening and prophylactic vaccination. There is an important medical need for disease modifying therapies to treat women with this terrible disease. For 2020, SEER databases project 13,800 new cases and 4,290 women dying from cervical cancer in the US. 

ISA101b is a clinical-stage immunotherapy targeting HPV16-induced diseases such as cervical and oropharyngeal cancer. It induces specific immune responses to the oncogenic E6 and E7 proteins of HPV16 and is based on ISA’s proprietary Synthetic Long Peptide (SLP®) technology.

“We are pleased to have Orphan Drug Designation for our ISA101b program targeting HPV16-positive cervical cancers,” said Kees Melief, Chief Scientific Officer of ISA Pharmaceuticals. “Our mission is to unleash the power of the patient’s own immune system to eradicate their cancer while maintaining optimal quality of life. We believe the best and most logical way to do this is through the use of SLPs. The metastatic form of cervical cancer has a particularly high unmet medical need with 5 year survival rates not going beyond 17%. The Orphan Drug Designation for ISA101b recognizes the urgent need for more efficacious treatment options for patients suffering from metastatic HPV16-positive cervical cancer.”

Vaccinatiebeleid van Blokhuis werpt vruchten af

Mooi nieuws voor onze VWS-staatssecretaris, zijn vaccinatiealliantie en Nederland in het algemeen: het ingezette vaccinatiebeleid werpt zijn vruchten af, en nieuwe verbeteringen liggen in het verschiet.

De stijgende vaccinatiegraad vormt een mooie eerste stap in de ambitie om zoveel mogelijk gezondheidswinst met vaccinatie te verzilveren. Dit najaar volgt de Raad voor Volksgezondheid en Samenleving bovendien op met een verkenning naar de toekomstbestendigheid van ons vaccinatiestelsel. HollandBIO heeft haar inspiratie voor een bij deze tijd passend systeem al met de Raad besproken. Wij kijken daarom reikhalzend uit naar het eindresultaat en de daaruit voortvloeiende vervolgstappen. En staatssecretaris Blokhuis? Die voelt zich gesterkt om zijn doortastend ingezette beleid voort te zetten, zo bericht hij de Tweede Kamer.

Dossier Dwanglicenties: de HollandBIO Weekly Leaks

Oh, de ironie: op de dag dat De Volkskrant een heel katern aan complottheorieën wijdt, “onthult” diezelfde krant dat de commissie dwanglicenties uit elkaar is geklapt door de stiekeme bemoeienis van HollandBIO. Helaas is de naakte waarheid een stuk minder spannend dan de chocoladeletter-kop doet vermoeden, en zoals wel vaker lang niet zo zwart-wit. Hoogste tijd voor een paar “onthullingen” van onze kant.

Onthulling 1: HollandBIO is een belangenvereniging

Voor ons, en hopelijk voor jullie ook, is het geen nieuws dat dat HollandBIO al ruim 6 jaar lang een belangenvereniging is. Voor De Volkskrant wel, die ons opvoert als farmalobbyisten. Onze lange tenen vinden dat dat niet helemaal de lading dekt, aangezien wij àlle biotech bedrijven vertegenwoordigen. Dus niet alleen grote farmaceuten, ook heel veel startups, scaleups en MKB’ers. Bedrijven die geneesmiddelen en vaccins ontwikkelen, slimme diagnostische testen, proefdiervrije innovaties, nieuwe plantenzaden, gecultiveerd vlees, en alle mogelijke duurzame biobased grondstoffen en ingrediënten. Maar oké, platgeslagen zijn we een lobbyclub. Als belangenvereniging voorzien we mensen en organisaties binnen en buiten de sector gevraagd en ongevraagd van informatie, analyses of redeneerlijnen. Bij de commissie dwanglicenties is dit ook het geval geweest. Dat is geen geheim. Sterker nog, het is de kern van wat we doen. Steeds meer organisaties vertrouwen op de kwaliteit van onze inbreng en onze bereidheid om samen tot oplossingen te komen. Zijn we trots op.

Onthulling 2: commissieleden en andere experts informeren zich

De Volkskrant schildert het af als een doodzonde, maar in de ogen van HollandBIO doen experts, of zij nu zitting nemen in commissies of niet, er juist goed aan om zich te informeren. Niemand kan immers alles weten, dus het toetsen van aannames en het opvragen van informatie helpt alleen maar om je een beter beeld te kunnen vormen van complexe onderwerpen en vraagstukken.

Onthulling 3: de commissie dwanglicenties was gedoemd te mislukken

Doorgaans vinden we het niet erg als onze invloed overschat wordt, maar laten we eerlijk zijn. In tegenstelling tot het beeld dat De Volkskrant en andere media, zoals de NOS (‘farmalobby probeerde advies over dwanglicenties te saboteren’), schetsen, was HollandBIO’s rol niet de reden dat de commissie dwanglicenties op een mislukking uitliep. De door de ministeries van EZK en VWS ingestelde commissie was, zoals de persoonlijke beschouwing van de commissievoorzitter ook al duidelijk maakt, gedoemd te mislukken. Al in een vroeg stadium bleken meningen en overtuigingen van de commissieleden over de (on)mogelijkheid en (on)wenselijkheid van de inzet van dwanglicenties mijlenver uiteen te lopen. Het ‘procedurele probleem’, zoals de rol van HollandBIO genoemd wordt, creëerde het perfecte excuus om de opdracht terug te geven en in plaats van verder blijven ploeteren naar een breder gedragen advies, een ‘persoonlijke beschouwing’ te schrijven. Wel zo makkelijk.

Onthulling 4: een dwanglicentie is noch sinecure, noch wondermiddel

Afgelopen donderdag ging -niet toevallig- op de drukste parlementaire dag van het jaar de brief over de uitkomsten uit de commissie dwanglicenties naar de Tweede Kamer. Dat is een beproefde ambtelijke methode om snel van een dossier af te zijn. Dat snappen we vanuit HollandBIO wel. In zowel de ‘persoonlijke beschouwing’ van de commissievoorzitter als uit de Kamerbrief wordt duidelijk dat dwanglicenties in alle opzichten – mag het, kan het, moeten we het willen – enorm veel haken en ogen kennen. Ze vragen per mogelijk geval om een complexe, integrale afweging. Zoveel nuance is voor een aantal commissieleden blijkbaar reden genoeg geweest om zelf een lijntje uit te gooien naar De Volkskrant, of om het dagblad desgevraagd op dit spoor te zetten, in een poging een groter podium voor hun eigen denkbeelden te krijgen.

Als bonus: drie scoops om ‘grootste onthullingen’ in de toekomst voor te zijn

De eerste: HollandBIO is niet tegen dwanglicenties. Het is een noodknop, een wettelijk verankerd instrument in te zetten wanneer de volksgezondheid in het geding is. Daarbij moet je eerder denken aan een pandemie, dan aan de situatie waarbij iemand vermoedt dat de QALY van een weesgeneesmiddel ook na geheime onderhandeling toch nog wat aan de hoge kant zou kunnen zijn. En juist zo’n laatste situatie was de aanleiding voor het instellen van de commissie. COVID-19 kwam pas later, toen de commissieleden al in onmin waren.

De tweede: HollandBIO zet zich in voor een ijzersterk innovatieklimaat. Vertrouw er dus maar op dat we, ook in de toekomst, iedereen gevraagd en ongevraagd met onze ideeën blijven bestoken om van Nederland een internationaal toonaangevende life sciences hub te maken.

En tot slot: alleen juristen profiteren van papieren tijgers. Van een florerende life sciences sector in Nederland, profiteert iedereen.
