Bij weinig wind ruik je in Delft de typische geur van gist. Al meer dan 150 jaar is Delft de stad waar gist geproduceerd wordt én waar biotechnologen onderzoeken hoe ze van gist steeds betere ‘celfabriekjes‘ kunnen maken. Door het genetisch materiaal van gist aan te passen kan je gist nuttige producten laten maken, zoals bio-ethanol, insuline of medicijnen tegen malaria. De manier waarop je deze aanpassingen doorvoert, blijkt telkens sneller en beter te kunnen met bijvoorbeeld CRISPR-Cas9. Onderzoekers van de TU Delft hebben nu echter met succes een kunstmatig chromosoom in gist gebouwd die aanpassingen nog makkelijker maakt. Ontketent deze doorbraak in gist een revolutie? De nieuwe mogelijkheden lijken eindeloos, bijvoorbeeld in duurzame productie van veel materialen en grondstoffen. HollandBIO is het dan ook volmondig eens met de woorden van professor Daran-Lapujade: “Wat biotechnologie betreft leven we echt in een fantastische tijd.”
De VSNU, NFU, KNAW, ZonMw en NWO zijn samen een nationaal initiatief gestart om de academische gemeenschap op een nieuwe manier te erkennen en waarderen. Het huidige systeem focust te veel op kwantitatieve indicatoren zoals publicaties en citaties en minder op de kwalitatieve indicatoren teamprestaties en maatschappelijke relevantie. Ook HollandBIO vindt dat het tijd is om de focus op maatschappelijke relevantie te versterken. Want de samenleving heeft alleen iets aan kennis die de maatschappij ook daadwerkelijk bereikt.
Nationaal initiatief om werk te maken van het nieuwe erkennen en waarderen
De position paper ‘Room for everyone’s talent’ van het consortium vormt de aftrap van het nationaal initiatief. Hierin pleiten zij voor een cultuurverandering die onderwijs, onderzoek en maatschappelijke relevantie in een betere balans brengt. HollandBIO is blij met dit geluid, omdat ondernemerschap, naast onderwijs en onderzoek, de belangrijke taak is van universiteiten. Door dit pleidooi krijgt de pijler ‘ondernemerschap’ meer aandacht.
Maatschappelijk relevant = excellente kennis omzetten tot producten
Recent onderzoekvan HollandBIO toont aan dat Nederland een goede kennispositie heeft. Helaas blijven de mate van bedrijvigheid en producten in ontwikkeling sterk achter. Alleen wanneer we kennis omzetten in producten die patiënten en consumenten bereiken, realiseren we maatschappelijke relevantie.

Galapagos NV (Euronext & Nasdaq: GLPG) welcomed the news that the National Institute for Health and Care Excellence (NICE) has issued a final appraisal determination (FAD) recommending the use of the daily oral pill, JYSELECA®▼ (filgotinib) on the National Health Service (NHS) in England for the treatment of eligible adult patients with moderate to severe active rheumatoid arthritis (RA).1 It is the first time in the UK that an advanced therapy has been recommended in people with moderate RA, offering thousands more the potential to achieve remission earlier – potentially slowing the irreversible damage and life-limiting symptoms RA can cause.2 RA is a degenerative auto-immune disease that can cause life-threatening complications.4 The sooner treatment begins, the better the chance of slowing disease progression.2 With thousands of people potentially eligible, the recommendation could help improve many lives as well as lessen the significant societal burden RA has in England.5,6
“We are delighted with the NICE recommendation for Jyseleca today. For patients with moderate to severe RA in England this decision represents a significant new opportunity and especially for those with moderate symptoms who can now receive an advanced treatment earlier,” said Onno van de Stolpe, Galapagos CEO.
Filgotinib is a once daily oral pill that can be given on its own (as a monotherapy) or used alongside another common RA medicine, called methotrexate.7 Eligible patients with moderate or severe RA will have responded inadequately to intensive therapy with 2 or more conventional disease-modifying antirheumatic drugs (DMARDs).1 Eligible patients with severe disease will also have wider access to filgotinib in line with criteria defined by NICE. Filgotinib is an advanced therapy which, in RA, is a term used to describe biologic DMARDs and targeted synthetic DMARDs.1
More than 400,000 people in the UK live with RA (around 380,000 in England), and it is recognised as a condition that can cause debilitating physical pain, affect mental health and require chronic care.2 Studies have shown that RA shortens life expectancy, with some estimates putting this at around 10 years.8 Nearly 50% of patients diagnosed with RA suffer from mental health issues with 1 in 6 people having a major depressive disorder.6,9 RA is also a significant burden on the UK economy. Around a third of people diagnosed with RA stop work within two years of diagnosis10 and the combined cost of workdays lost due to osteoarthritis and RA in the UK was estimated at £2.58 billion in 2017 – estimated to rise to £3.43 billion by 2030.5
NICE guidance covers England. Wales and Northern Ireland are expected to follow the guidance with timelines for implementation currently under consideration. Filgotinib will be reviewed separately by the Scottish Medicines Consortium for use on the NHS in Scotland.
Under a new arrangement between Gilead and Galapagos, announced in December 2020, Galapagos will assume sole responsibility for filgotinib in Europe, including the UK. Through a phased transition the majority of activities supporting filgotinib in Europe are expected to be assumed by Galapagos by the end of 2021.

ISA Pharmaceuticals B.V. today announced that the first patient has been dosed with its lead product ISA101b in a Phase II trial conducted and sponsored by UPMC Hillman Cancer Center, the University of Pittsburgh, PA, and supported by Merck Sharp & Dohme Corp and ISA Pharmaceuticals B.V.
The study in previously untreated patients with intermediate-risk head and neck cancer associated with human papilloma virus subtype 16 (HPV-16) will evaluate the efficacy of ISA101b and pembrolizumab (Keytruda®) in combination with cisplatin and radiotherapy. The principal investigator is Robert L. Ferris MD, PhD, Director of UPMC Hillman Cancer Center in Pittsburgh and Associate Vice Chancellor for Cancer Research at the University of Pittsburgh School of Medicine. Dr. Ferris is a paid member of the US Scientific Advisory Board for the Merck Sharp & Dohme Corp. This study was supported in part by a research grant from the Investigator-Initiated Studies Program of Merck Sharp & Dohme Corp.
The investigator-initiated, single-arm, open-label ‘Phase II Study Evaluating HPV-16 vaccination (ISA101b) and Pembrolizumab plus Cisplatin Chemoradiotherapy for Intermediate Risk HPV-16 associated Head and Neck Squamous Cell Carcinoma’ aims to enroll 50 patients with end of enrollment expected in late 2022.
The primary endpoint of the trial is 2-year progression-free survival (PFS) in patients with newly diagnosed, local-regionally advanced, intermediate risk HPV-associated head and neck cancer.
“For head and neck cancer patients at risk for tumor recurrence, chemo- and radiation therapy are being combined with PD-1 immune checkpoint inhibitors,” said Dr. Ferris. “We seek to leverage the effect of immunotherapy to enhance T cell specific immune responses against HPV, to provide for a more profound and durable anti-tumor effect. To explore this, we have chosen the synthetic long peptide (SLP) therapeutic HPV immunotherapy ISA101b. ISA Pharmaceuticals has demonstrated that HPV, as the predominant cause of this type of cancer, harbours powerful antigens against which virus-specific T cells will mount an attack on the cancer, and we will attempt to enhance this benefit using anti-PD1 pembrolizimab.”
“Currently immunotherapies combined with additional treatments are emerging as the most promising approach in oncology,” said Prof. Cornelis Melief, Chief Scientific Officer of ISA Pharmaceuticals. “Consistent with this, we have demonstrated in multiple trials that ISA101b is highly synergistic with various complementary cancer treatments. Our product has shown very promising results in Phase II combination trials with standard chemotherapy1 and the checkpoint inhibitor nivolumab2. This new trial will provide valuable insights into the effect of immunomodulation in the early stage adjuvant setting, in addition to our ongoing randomized Phase II ISA101b trial with cemiplimab in late stage oropharyngeal cancer. U.S. consumers may be able to dine on cell-based fish as soon as this year. BlueNalu Inc. raised $60 million as the San Diego-based startup seeks to open a trial factory and start testing cultured products at food outlets across the U.S. by year-end. The company is working on completing a regulatory review with the Food and Drug Administration, which could see it become the first to start selling cell-based fish in the $200 billion global seafood market.
Cellular agriculture, which aims to make meat or fish production more environmentally sustainable, is slowly making its way toward consumer plates. A string of startups are building pilot plants to test meat produced in factories for the first time, with the sector attracting record venture-capital funding last year. Eat Just Inc., another California startup, is already selling laboratory-created chicken in Singapore after getting approval last month.
“We have an awful lot of interest from leading food-service establishments across the U.S.,” BlueNalu Chief Executive Officer Lou Cooperhouse said in an interview, declining to name companies. “They will be iconic seafood restaurants that are well known for great, fresh seafood. But also more like fast-casual.”
The $60 million raised in debt financing marked the largest funding round so far in the cell-based seafood sector, BlueNalu said in a statement. Investors include Rage Capital, Agronomics Ltd. and Lewis & Clark AgriFood, it said.
The company plans to first introduce mahi mahi later this year, followed by bluefin tuna. Its almost 40,000-square-foot trial factory will be located in San Diego and is expected to be completed during the third quarter.

Loxo Oncology at Lilly, a research and development group of Eli Lilly and Company (NYSE: LLY), and Merus N.V. (NASDAQ: MRUS), a clinical-stage oncology company developing multi-specific antibodies, today announced a research collaboration and exclusive license agreement that will leverage Merus’ proprietary Biclonics® platform along with the scientific and rational drug design expertise of Loxo Oncology at Lilly to research and develop up to three CD3-engaging T-cell re-directing bispecific antibody therapies.
Under the terms of the agreement, Merus will lead discovery and early stage research activities while Loxo Oncology at Lilly will be responsible for additional research, development and commercialization activities. Merus will receive an upfront cash payment of $40 million, as well as an equity investment by Lilly of $20 million in Merus common shares. Merus is also eligible to receive up to $540 million in potential development and commercialization milestones per product, for a total of up to approximately $1.6 billion for three products, as well as tiered royalties ranging from the mid-single to low-double digits on product sales should Lilly successfully commercialize a therapy from the collaboration.
“CD3-engaging bispecific antibodies are rapidly becoming one of the most transformative immune-modulating modalities used to treat cancer. We expect these therapies will become an important component of the Loxo Oncology at Lilly biologics strategy,” said Jacob Van Naarden, chief operating officer of Loxo Oncology at Lilly. “Merus has built a differentiated platform and one that we believe can enable us to create bispecific antibody therapies with wider therapeutic indexes than those available today. We look forward to working closely with Merus to develop new potential medicines for patients with cancer.”

This transaction is subject to customary closing conditions. This transaction will be reflected in Lilly’s reported results and financial guidance according to Generally Accepted Accounting Principles (GAAP). There will be no change to Lilly’s 2021 non-GAAP earnings per share guidance as a result of this transaction.
De kogel is door de kerk, althans, voor het Zorginstituut. Afgelopen week adviseerde het Zorginstituut het ministerie van VWS om het medicijn mexiletine (Namuscla®) niet op te nemen in het geneesmiddelenvergoedingssysteem. Het laatste woord is daarmee aan de minister. HollandBIO begrijpt de kritiek op de prijsstelling van het weesgeneesmiddel. Toch vinden wij het onwenselijk dat het Zorginstituut met dit advies het wettelijk kader inruilt voor een – per definitie subjectief – moreel kompas. We pleitten er eerder voor en doen dat nu weer: ongewenst gebruik van de weesgeneesmiddelenverordening voorkom je aan de bron, door aanscherping van die regels, ofwel door geneesmiddelen die met succes off-label gebruikt worden, als de wiedeweerga zelf te registreren.
Alweer ruim twee jaar geleden berichtte de Volkskrant over “medicijnkaper” Lupin Pharmacueticals. De geneesmiddelenfabrikant kreeg een handelsvergunning van het EMA voor het gebruik van een oud geneesmiddel, mexiletine (Namuscla®), voor een nieuwe indicatie, een zeldzame spierziekte. Het bedrijf maakte daarbij gebruik van de stimuleringsmaatregelen voor de ontwikkeling en registratie van weesgeneesmiddelen. Of volgens velen, misbruik. Hoewel de prijs in Nederland destijds nog niet bekend was, werd gevreesd dat Lupin dankzij de verkregen exclusiviteit de prijs fors zou opdrijven, waardoor de behandeling van patiënten vele malen duurder werd dan bij het gebruik van de tot dan toe naar tevredenheid gebruikte, maar ongeregistreerde, apotheekbereidingen.
Die vrees werd bewaarheid. Lupin introduceerde het geneesmiddel in Nederland met een buitenproportioneel prijskaartje. Ook na een door oud-minister Bruins bedongen korting op de apotheekinkoopprijs van 40%, bleef Namuscla® vele malen duurder dan de apotheekbereiding. Hoewel het Zorginstituut in principe positief oordeelt over opname op bijlage 1B van het GVS, adviseert het instituut de minister het middel niet in het basispakket op te nemen en in plaats daarvan de apotheekbereidingen te vergoeden. Namuscla® is simpelweg te duur, aldus het Zorginstituut, dat in het begeleidende persbericht laat optekenen dat: “de prijsstelling in geen verhouding staat tot de inspanningen om Namuscla® geregistreerd te krijgen”.
Een opvallend advies, dat op sympathie kan rekenen. Ook HollandBIO begrijpt de frustratie en onmacht van de situatie. Een geneesmiddel wordt gekaapt en er rest ons niets anders dan of onderhandelen en betalen, of patiënten in de kou laten staan. Maar toch. Het Zorginstituut oordeelt dat Namuscla® noodzakelijk is, behoort tot de stand der wetenschap en praktijk en dat gezien de budgetimpact van minder dan 10 miljoen euro een farmaco-economische beoordeling niet aan de orde is. Daarmee zouden toch alle seinen op groen moeten staan voor opname in het basispakket? De “geleverde inspanning voor het verkrijgen van een registratie” is immers geen criterium voor vergoeding, en in de regel is een geregistreerde behandeling preferent ten opzichte van een bereiding. Wat is een registratie waard? Hoeveel inspanning verantwoordt welke prijsverhoging? Waar ligt de grens? En is dat te objectiveren? Het verleden wijst immers uit dat ook een prijsstijging van enkele euro’s na registratie collectieve woede kan aanwakkeren. Met dit advies van het Zorginstituut dreigen we een helder wettelijk kader in te ruilen voor een grijs gebied van morele willekeur.
Moeten we dan maar lijdzaam toezien hoe medicijnkapers de solidariteit van ons zorgstelsel ondermijnen? Tuurlijk niet. Wees die handige jongens te slim af, en regel zelf die registratie, stelden we eerder voor in ons stuk Voorkom een rel, registreer snel. Daarnaast biedt de huidige evaluatie van de weesgeneesmiddelenverordening de uitgelezen kans om, op basis van twintig jaar ervaring de stimuleringsmaatregelen verder te scherpen, innovatie een impuls te geven en en passant medicijnkapers een stok in de wielen te gooien. Laten we samen optrekken om ongewenst gebruik onmogelijk maken, en belangrijker nog: de stimuleringsmaatregelen zo in te richten dat we op termijn elke zeldzame ziekte patiënt, overal in Europa, toegang kunnen geven tot een passende behandeling.

Goed nieuws in het AD vanochtend! Artsen, het Zorginstituut en zorgverzekeraars starten met het nieuwe Drug Access Protocol (DAP): individuele vergoeding van dure geneesmiddelen voor patiënten met zeldzame vormen van kanker. Klinkt als de DRUP-studie, maar dan voor nieuwe kankergeneesmiddelen. HollandBIO juicht het initiatief van harte toe, omdat de toegang voor patiënten sneller en beter wordt.
De eerste fabrikant die de DAPpere stap neemt om mee te werken is Sanofi, met het middel cemiplimab. Ongeveer zestig tot zeventig patiënten per jaar hebben baat bij dit middel tegen plaveiselcelcarcinoom van de huid. De afspraak die is gemaakt houdt in dat cemiplimab vier maanden op kosten van de fabrikant gebruikt wordt voor het behandelen van patiënten. Als na vier maanden blijkt dat de tumor van de patiënt gekrompen of gelijk is gebleven, worden alsnog alle kosten vergoed uit het basispakket. Het mooie van dit initiatief is dat het risico bij de fabrikant ligt. Zij krijgen er op hun beurt waardevolle data over de werking van hun medicijn in de ‘echte wereld’ voor terug.
HollandBIO is blij dat de vergoeding van deze geneesmiddelen op individuele basis wordt aangepakt. Zoals Haiko Bloemendal (NVMO) tegen de NOS zegt: “de grote winst is dat patiënten die toevallig een zeldzamere vorm van kanker hebben niet de dupe zijn omdat het langer duurt voor er een besluit wordt genomen over de vergoeding van een nieuw medicijn”. Het DAP is een mooie stap in de goede richting om innovatieve geneesmiddelen innovatief te beoordelen. En goed voorbeeld doet volgen. HollandBIO hoopt op een sneeuwbaleffect, want ook andere patiënten met zeldzame ziekten hebben baat bij individuele vergoeding. Zo zorgen we er met elkaar voor dat elke patiënt sneller en beter de juiste behandeling op het juiste moment krijgt én vergoeden we alleen effectieve behandelingen. Werk aan de winkel dus!

De komst van steeds meer gepersonaliseerde behandelingen, waaronder cel- en gentherapieën, bieden hoop voor ernstig zieke patiënten. Maar de ontwikkeling én productie van deze geavanceerde therapieën is kostbaar, risicovol en duurt lang. Technologische innovaties bieden kansen voor efficiëntere geneesmiddelontwikkeling en stimuleren op deze manier gezondheidswinst en betaalbaarheid.
HollandBIO zet een paar illustratieve voorbeelden van het effect van technologische innovatie op de geneesmiddelontwikkeling vers van de pers voor je op een rijtje. Zo kan door de inzet van CRISPR-technologie de screening en validatie van nieuwe drug targets, als ook de ontwikkeling van relevante cel- en diermodellen efficiënter. Kunstmatige intelligentie helpt bij het zoeken naar nieuwe antibiotica en met cryo-elektronmicroscopie vond farmaceut Janssen het juiste spike-eiwit voor hun coronavaccin.
En niet alleen de ontwikkeling, ook de productie van geneesmiddelen kan baat hebben bij technologie. Zo pleitte Liesbet Lagae afgelopen week in de Volkskrant voor het automatiseren van het productieproces van CAR-T-celtherapie. Het doel: meer revolutionaire kankerbehandelingen toegankelijk voor de patiënt. Lagae ziet een leidende rol voor Europa in deze automatisering en pleit daarom voor interdisciplinaire samenwerking op dit vlak.
Ook HollandBIO droomt van een toekomst waarin gezondheid op maat de standaard is. Ondanks dit wenkend perspectief blijkt de route van lab naar praktijk voor deze technologische innovaties hobbelig. De manier waarop je deze aanpassingen doorvoert, blijkt telkens sneller en beter te kunnen met bijvoorbeeld CRISPR-Cas9. Onderzoekers van de TU Delft hebben nu echter met succes een kunstmatig chromosoom in gist gebouwd die aanpassingen nog makkelijker maakt. Ontketent deze doorbraak in gist een revolutie? De nieuwe mogelijkheden lijken eindeloos, bijvoorbeeld in duurzame productie van veel materialen en grondstoffen. HollandBIO is het dan ook volmondig eens met de woorden van professor Daran-Lapujade: “Wat biotechnologie betreft leven we echt in een fantastische tijd.”

Op naar meer wetenschappelijke relevantie, valorisatie en ondernemerschap in de academie
IJzersterk innovatieklimaat, Innovatieklimaat, UitgelichtDe VSNU, NFU, KNAW, ZonMw en NWO zijn samen een nationaal initiatief gestart om de academische gemeenschap op een nieuwe manier te erkennen en waarderen. Het huidige systeem focust te veel op kwantitatieve indicatoren zoals publicaties en citaties en minder op de kwalitatieve indicatoren teamprestaties en maatschappelijke relevantie. Ook HollandBIO vindt dat het tijd is om de focus op maatschappelijke relevantie te versterken. Want de samenleving heeft alleen iets aan kennis die de maatschappij ook daadwerkelijk bereikt.
Nationaal initiatief om werk te maken van het nieuwe erkennen en waarderen
De position paper ‘Room for everyone’s talent’ van het consortium vormt de aftrap van het nationaal initiatief. Hierin pleiten zij voor een cultuurverandering die onderwijs, onderzoek en maatschappelijke relevantie in een betere balans brengt. HollandBIO is blij met dit geluid, omdat ondernemerschap, naast onderwijs en onderzoek, de belangrijke taak is van universiteiten. Door dit pleidooi krijgt de pijler ‘ondernemerschap’ meer aandacht.
Maatschappelijk relevant = excellente kennis omzetten tot producten
Recent onderzoek van HollandBIO toont aan dat Nederland een goede kennispositie heeft. Helaas blijven de mate van bedrijvigheid en producten in ontwikkeling sterk achter. Alleen wanneer we kennis omzetten in producten die patiënten en consumenten bereiken, realiseren we maatschappelijke relevantie.
Hoe kunnen we dan maatschappelijke relevantie aanbrengen bij het erkennen en waarderen van academici? HollandBIO ziet drie relevante aanbevelingen:

NICE recommends Jyseleca® ▼(filgotinib) on NHS in landmark decision for rheumatoid arthritis
GezondheidGalapagos NV (Euronext & Nasdaq: GLPG) welcomed the news that the National Institute for Health and Care Excellence (NICE) has issued a final appraisal determination (FAD) recommending the use of the daily oral pill, JYSELECA®▼ (filgotinib) on the National Health Service (NHS) in England for the treatment of eligible adult patients with moderate to severe active rheumatoid arthritis (RA).1 It is the first time in the UK that an advanced therapy has been recommended in people with moderate RA, offering thousands more the potential to achieve remission earlier – potentially slowing the irreversible damage and life-limiting symptoms RA can cause.2 RA is a degenerative auto-immune disease that can cause life-threatening complications.4 The sooner treatment begins, the better the chance of slowing disease progression.2 With thousands of people potentially eligible, the recommendation could help improve many lives as well as lessen the significant societal burden RA has in England.5,6
“We are delighted with the NICE recommendation for Jyseleca today. For patients with moderate to severe RA in England this decision represents a significant new opportunity and especially for those with moderate symptoms who can now receive an advanced treatment earlier,” said Onno van de Stolpe, Galapagos CEO.
Filgotinib is a once daily oral pill that can be given on its own (as a monotherapy) or used alongside another common RA medicine, called methotrexate.7 Eligible patients with moderate or severe RA will have responded inadequately to intensive therapy with 2 or more conventional disease-modifying antirheumatic drugs (DMARDs).1 Eligible patients with severe disease will also have wider access to filgotinib in line with criteria defined by NICE. Filgotinib is an advanced therapy which, in RA, is a term used to describe biologic DMARDs and targeted synthetic DMARDs.1
More than 400,000 people in the UK live with RA (around 380,000 in England), and it is recognised as a condition that can cause debilitating physical pain, affect mental health and require chronic care.2 Studies have shown that RA shortens life expectancy, with some estimates putting this at around 10 years.8 Nearly 50% of patients diagnosed with RA suffer from mental health issues with 1 in 6 people having a major depressive disorder.6,9 RA is also a significant burden on the UK economy. Around a third of people diagnosed with RA stop work within two years of diagnosis10 and the combined cost of workdays lost due to osteoarthritis and RA in the UK was estimated at £2.58 billion in 2017 – estimated to rise to £3.43 billion by 2030.5
NICE guidance covers England. Wales and Northern Ireland are expected to follow the guidance with timelines for implementation currently under consideration. Filgotinib will be reviewed separately by the Scottish Medicines Consortium for use on the NHS in Scotland.
Under a new arrangement between Gilead and Galapagos, announced in December 2020, Galapagos will assume sole responsibility for filgotinib in Europe, including the UK. Through a phased transition the majority of activities supporting filgotinib in Europe are expected to be assumed by Galapagos by the end of 2021.

First Patient Treated in Phase II Combination Trial of ISA Pharmaceuticals’ Lead Immunotherapy ISA101b with Keytruda®
GezondheidISA Pharmaceuticals B.V. today announced that the first patient has been dosed with its lead product ISA101b in a Phase II trial conducted and sponsored by UPMC Hillman Cancer Center, the University of Pittsburgh, PA, and supported by Merck Sharp & Dohme Corp and ISA Pharmaceuticals B.V.
The study in previously untreated patients with intermediate-risk head and neck cancer associated with human papilloma virus subtype 16 (HPV-16) will evaluate the efficacy of ISA101b and pembrolizumab (Keytruda®) in combination with cisplatin and radiotherapy. The principal investigator is Robert L. Ferris MD, PhD, Director of UPMC Hillman Cancer Center in Pittsburgh and Associate Vice Chancellor for Cancer Research at the University of Pittsburgh School of Medicine. Dr. Ferris is a paid member of the US Scientific Advisory Board for the Merck Sharp & Dohme Corp. This study was supported in part by a research grant from the Investigator-Initiated Studies Program of Merck Sharp & Dohme Corp.
The investigator-initiated, single-arm, open-label ‘Phase II Study Evaluating HPV-16 vaccination (ISA101b) and Pembrolizumab plus Cisplatin Chemoradiotherapy for Intermediate Risk HPV-16 associated Head and Neck Squamous Cell Carcinoma’ aims to enroll 50 patients with end of enrollment expected in late 2022.
The primary endpoint of the trial is 2-year progression-free survival (PFS) in patients with newly diagnosed, local-regionally advanced, intermediate risk HPV-associated head and neck cancer.
“For head and neck cancer patients at risk for tumor recurrence, chemo- and radiation therapy are being combined with PD-1 immune checkpoint inhibitors,” said Dr. Ferris. “We seek to leverage the effect of immunotherapy to enhance T cell specific immune responses against HPV, to provide for a more profound and durable anti-tumor effect. To explore this, we have chosen the synthetic long peptide (SLP) therapeutic HPV immunotherapy ISA101b. ISA Pharmaceuticals has demonstrated that HPV, as the predominant cause of this type of cancer, harbours powerful antigens against which virus-specific T cells will mount an attack on the cancer, and we will attempt to enhance this benefit using anti-PD1 pembrolizimab.”
“Currently immunotherapies combined with additional treatments are emerging as the most promising approach in oncology,” said Prof. Cornelis Melief, Chief Scientific Officer of ISA Pharmaceuticals. “Consistent with this, we have demonstrated in multiple trials that ISA101b is highly synergistic with various complementary cancer treatments. Our product has shown very promising results in Phase II combination trials with standard chemotherapy1 and the checkpoint inhibitor nivolumab2. This new trial will provide valuable insights into the effect of immunomodulation in the early stage adjuvant setting, in addition to our ongoing randomized Phase II ISA101b trial with cemiplimab in late stage oropharyngeal cancer. Cultured Fish May Go From Lab to U.S. Plates This Year

BlueNalu Inc. raised $60 million as the San Diego-based startup seeks to open a trial factory and start testing cultured products at food outlets across the U.S. by year-end. The company is working on completing a regulatory review with the Food and Drug Administration, which could see it become the first to start selling cell-based fish in the $200 billion global seafood market.
Cellular agriculture, which aims to make meat or fish production more environmentally sustainable, is slowly making its way toward consumer plates. A string of startups are building pilot plants to test meat produced in factories for the first time, with the sector attracting record venture-capital funding last year. Eat Just Inc., another California startup, is already selling laboratory-created chicken in Singapore after getting approval last month.
“We have an awful lot of interest from leading food-service establishments across the U.S.,” BlueNalu Chief Executive Officer Lou Cooperhouse said in an interview, declining to name companies. “They will be iconic seafood restaurants that are well known for great, fresh seafood. But also more like fast-casual.”
The $60 million raised in debt financing marked the largest funding round so far in the cell-based seafood sector, BlueNalu said in a statement. Investors include Rage Capital, Agronomics Ltd. and Lewis & Clark AgriFood, it said.
The company plans to first introduce mahi mahi later this year, followed by bluefin tuna. Its almost 40,000-square-foot trial factory will be located in San Diego and is expected to be completed during the third quarter.

Lilly and Merus NV Announce Collaboration to Discover Novel T-Cell Re-Directing Bispecific Antibodies
InnovatieklimaatLoxo Oncology at Lilly, a research and development group of Eli Lilly and Company (NYSE: LLY), and Merus N.V. (NASDAQ: MRUS), a clinical-stage oncology company developing multi-specific antibodies, today announced a research collaboration and exclusive license agreement that will leverage Merus’ proprietary Biclonics® platform along with the scientific and rational drug design expertise of Loxo Oncology at Lilly to research and develop up to three CD3-engaging T-cell re-directing bispecific antibody therapies.
Under the terms of the agreement, Merus will lead discovery and early stage research activities while Loxo Oncology at Lilly will be responsible for additional research, development and commercialization activities. Merus will receive an upfront cash payment of $40 million, as well as an equity investment by Lilly of $20 million in Merus common shares. Merus is also eligible to receive up to $540 million in potential development and commercialization milestones per product, for a total of up to approximately $1.6 billion for three products, as well as tiered royalties ranging from the mid-single to low-double digits on product sales should Lilly successfully commercialize a therapy from the collaboration.
“CD3-engaging bispecific antibodies are rapidly becoming one of the most transformative immune-modulating modalities used to treat cancer. We expect these therapies will become an important component of the Loxo Oncology at Lilly biologics strategy,” said Jacob Van Naarden, chief operating officer of Loxo Oncology at Lilly. “Merus has built a differentiated platform and one that we believe can enable us to create bispecific antibody therapies with wider therapeutic indexes than those available today. We look forward to working closely with Merus to develop new potential medicines for patients with cancer.”

This transaction is subject to customary closing conditions. This transaction will be reflected in Lilly’s reported results and financial guidance according to Generally Accepted Accounting Principles (GAAP). There will be no change to Lilly’s 2021 non-GAAP earnings per share guidance as a result of this transaction.

De prijs van registratie: wie biedt?
Gezondheid op maat, Gezondheid, UitgelichtDe kogel is door de kerk, althans, voor het Zorginstituut. Afgelopen week adviseerde het Zorginstituut het ministerie van VWS om het medicijn mexiletine (Namuscla®) niet op te nemen in het geneesmiddelenvergoedingssysteem. Het laatste woord is daarmee aan de minister. HollandBIO begrijpt de kritiek op de prijsstelling van het weesgeneesmiddel. Toch vinden wij het onwenselijk dat het Zorginstituut met dit advies het wettelijk kader inruilt voor een – per definitie subjectief – moreel kompas. We pleitten er eerder voor en doen dat nu weer: ongewenst gebruik van de weesgeneesmiddelenverordening voorkom je aan de bron, door aanscherping van die regels, ofwel door geneesmiddelen die met succes off-label gebruikt worden, als de wiedeweerga zelf te registreren.
Alweer ruim twee jaar geleden berichtte de Volkskrant over “medicijnkaper” Lupin Pharmacueticals. De geneesmiddelenfabrikant kreeg een handelsvergunning van het EMA voor het gebruik van een oud geneesmiddel, mexiletine (Namuscla®), voor een nieuwe indicatie, een zeldzame spierziekte. Het bedrijf maakte daarbij gebruik van de stimuleringsmaatregelen voor de ontwikkeling en registratie van weesgeneesmiddelen. Of volgens velen, misbruik. Hoewel de prijs in Nederland destijds nog niet bekend was, werd gevreesd dat Lupin dankzij de verkregen exclusiviteit de prijs fors zou opdrijven, waardoor de behandeling van patiënten vele malen duurder werd dan bij het gebruik van de tot dan toe naar tevredenheid gebruikte, maar ongeregistreerde, apotheekbereidingen.
Die vrees werd bewaarheid. Lupin introduceerde het geneesmiddel in Nederland met een buitenproportioneel prijskaartje. Ook na een door oud-minister Bruins bedongen korting op de apotheekinkoopprijs van 40%, bleef Namuscla® vele malen duurder dan de apotheekbereiding. Hoewel het Zorginstituut in principe positief oordeelt over opname op bijlage 1B van het GVS, adviseert het instituut de minister het middel niet in het basispakket op te nemen en in plaats daarvan de apotheekbereidingen te vergoeden. Namuscla® is simpelweg te duur, aldus het Zorginstituut, dat in het begeleidende persbericht laat optekenen dat: “de prijsstelling in geen verhouding staat tot de inspanningen om Namuscla® geregistreerd te krijgen”.
Een opvallend advies, dat op sympathie kan rekenen. Ook HollandBIO begrijpt de frustratie en onmacht van de situatie. Een geneesmiddel wordt gekaapt en er rest ons niets anders dan of onderhandelen en betalen, of patiënten in de kou laten staan. Maar toch. Het Zorginstituut oordeelt dat Namuscla® noodzakelijk is, behoort tot de stand der wetenschap en praktijk en dat gezien de budgetimpact van minder dan 10 miljoen euro een farmaco-economische beoordeling niet aan de orde is. Daarmee zouden toch alle seinen op groen moeten staan voor opname in het basispakket? De “geleverde inspanning voor het verkrijgen van een registratie” is immers geen criterium voor vergoeding, en in de regel is een geregistreerde behandeling preferent ten opzichte van een bereiding. Wat is een registratie waard? Hoeveel inspanning verantwoordt welke prijsverhoging? Waar ligt de grens? En is dat te objectiveren? Het verleden wijst immers uit dat ook een prijsstijging van enkele euro’s na registratie collectieve woede kan aanwakkeren. Met dit advies van het Zorginstituut dreigen we een helder wettelijk kader in te ruilen voor een grijs gebied van morele willekeur.
Moeten we dan maar lijdzaam toezien hoe medicijnkapers de solidariteit van ons zorgstelsel ondermijnen? Tuurlijk niet. Wees die handige jongens te slim af, en regel zelf die registratie, stelden we eerder voor in ons stuk Voorkom een rel, registreer snel. Daarnaast biedt de huidige evaluatie van de weesgeneesmiddelenverordening de uitgelezen kans om, op basis van twintig jaar ervaring de stimuleringsmaatregelen verder te scherpen, innovatie een impuls te geven en en passant medicijnkapers een stok in de wielen te gooien. Laten we samen optrekken om ongewenst gebruik onmogelijk maken, en belangrijker nog: de stimuleringsmaatregelen zo in te richten dat we op termijn elke zeldzame ziekte patiënt, overal in Europa, toegang kunnen geven tot een passende behandeling.

Nieuwe kankergeneesmiddelen sneller bij de patiënt
Gezondheid op maat, GezondheidGoed nieuws in het AD vanochtend! Artsen, het Zorginstituut en zorgverzekeraars starten met het nieuwe Drug Access Protocol (DAP): individuele vergoeding van dure geneesmiddelen voor patiënten met zeldzame vormen van kanker. Klinkt als de DRUP-studie, maar dan voor nieuwe kankergeneesmiddelen. HollandBIO juicht het initiatief van harte toe, omdat de toegang voor patiënten sneller en beter wordt.
De eerste fabrikant die de DAPpere stap neemt om mee te werken is Sanofi, met het middel cemiplimab. Ongeveer zestig tot zeventig patiënten per jaar hebben baat bij dit middel tegen plaveiselcelcarcinoom van de huid. De afspraak die is gemaakt houdt in dat cemiplimab vier maanden op kosten van de fabrikant gebruikt wordt voor het behandelen van patiënten. Als na vier maanden blijkt dat de tumor van de patiënt gekrompen of gelijk is gebleven, worden alsnog alle kosten vergoed uit het basispakket. Het mooie van dit initiatief is dat het risico bij de fabrikant ligt. Zij krijgen er op hun beurt waardevolle data over de werking van hun medicijn in de ‘echte wereld’ voor terug.
HollandBIO is blij dat de vergoeding van deze geneesmiddelen op individuele basis wordt aangepakt. Zoals Haiko Bloemendal (NVMO) tegen de NOS zegt: “de grote winst is dat patiënten die toevallig een zeldzamere vorm van kanker hebben niet de dupe zijn omdat het langer duurt voor er een besluit wordt genomen over de vergoeding van een nieuw medicijn”. Het DAP is een mooie stap in de goede richting om innovatieve geneesmiddelen innovatief te beoordelen. En goed voorbeeld doet volgen. HollandBIO hoopt op een sneeuwbaleffect, want ook andere patiënten met zeldzame ziekten hebben baat bij individuele vergoeding. Zo zorgen we er met elkaar voor dat elke patiënt sneller en beter de juiste behandeling op het juiste moment krijgt én vergoeden we alleen effectieve behandelingen. Werk aan de winkel dus!
Technologische innovaties tornen aan het systeem van geneesmiddelontwikkeling en dat is top
Gezondheid op maat, Gezondheid, UitgelichtDe komst van steeds meer gepersonaliseerde behandelingen, waaronder cel- en gentherapieën, bieden hoop voor ernstig zieke patiënten. Maar de ontwikkeling én productie van deze geavanceerde therapieën is kostbaar, risicovol en duurt lang. Technologische innovaties bieden kansen voor efficiëntere geneesmiddelontwikkeling en stimuleren op deze manier gezondheidswinst en betaalbaarheid.
HollandBIO zet een paar illustratieve voorbeelden van het effect van technologische innovatie op de geneesmiddelontwikkeling vers van de pers voor je op een rijtje. Zo kan door de inzet van CRISPR-technologie de screening en validatie van nieuwe drug targets, als ook de ontwikkeling van relevante cel- en diermodellen efficiënter. Kunstmatige intelligentie helpt bij het zoeken naar nieuwe antibiotica en met cryo-elektronmicroscopie vond farmaceut Janssen het juiste spike-eiwit voor hun coronavaccin.
En niet alleen de ontwikkeling, ook de productie van geneesmiddelen kan baat hebben bij technologie. Zo pleitte Liesbet Lagae afgelopen week in de Volkskrant voor het automatiseren van het productieproces van CAR-T-celtherapie. Het doel: meer revolutionaire kankerbehandelingen toegankelijk voor de patiënt. Lagae ziet een leidende rol voor Europa in deze automatisering en pleit daarom voor interdisciplinaire samenwerking op dit vlak.
Ook HollandBIO droomt van een toekomst waarin gezondheid op maat de standaard is. Ondanks dit wenkend perspectief blijkt de route van lab naar praktijk voor deze technologische innovaties hobbelig.
