EMA zoekt sprekers voor MKB-dag

Op 29 oktober organiseert het Europees medicijnen agentschap (EMA) een dag speciaal voor start-ups, kleine en middelgrote bedrijven (MKB). Onderwerpen die de revue zullen passeren zijn de stimuleringsmaatregelen voor weesgeneesmiddelen en geneesmiddelen voor kinderen, mogelijkheden voor versnelde toelating en toelating onder voorwaarden, de nieuwe wetgeving op het gebied van medische hulpmiddelen en de gevolgen van de Brexit. EMA is op zoek naar sprekers die hun ervaring met een van deze onderwerpen willen delen of een sessie willen modereren. Heb je ervaring, interesse en werk je bij een SME?

Kite leased a new facility in the Netherlands

Kite has leased a new facility in the Netherlands to engineer cell therapies in Europe. The 117,000 square-foot site in Hoofddorp (SEGRO Park Amsterdam Airport) will enable Kite to efficiently manufacture and deliver its cell therapies to people living with cancer in Europe and will provide more than 300 new jobs when fully operational in 2020.

The facility will engineer and produce innovative cell therapies, including axicabtagene ciloleucel, a Chimeric Antigen Receptor T cell (CAR T) therapy that is currently under review by the European Medicines Agency and which is approved in the United States as Yescarta.

“We are pleased to be leading a new frontier of cancer innovation that is bringing hope for people living with cancer,” commented John F. Milligan, PhD, Gilead’s President and Chief Executive Officer. “This new European manufacturing facility will enable personalized cell therapies to be manufactured in closer geographic proximity to the patients who will receive them, potentially shortening the turnaround time for people who urgently need care.”

In addition to the Netherlands facility, Kite has recently purchased a new building in Santa Monica from Astellas Pharma Inc. that will be used for cell therapy research, development and the expansion of clinical manufacturing capabilities, and has leased a 26,000 square-foot facility in Gaithersburg, Maryland. The Maryland site will support the work of a new Cooperative Research and Development Agreement (CRADA) with the National Cancer Institute (NCI) to develop adoptive cell therapies targeting patient-specific tumor neoantigens. Neoantigens are mutations found on the surface of cancer cells that are unique to each person and tumor, offering the potential for more targeted antitumor activity.

Patient Engagement Guide helpt bedrijven de patiënt centraal te stellen

Den Haag, 14 mei 2018 – admedicum en HollandBIO hebben tijdens de European Conference on Rare Diseases and Orphan Products (ECRD) te Wenen de Patient Engagement Guide (PEG) gelanceerd. Dit praktische online handboek helpt bedrijven de stem van de patiënt een structurele en sturende rol in hun organisatie te geven. Betrokkenheid van patiënten in elke fase van de geneesmiddelontwikkeling is cruciaal voor een sneller en beter geneesmiddel ontwikkeltraject.

De ECRD is de grootste Europese conferentie voor zeldzame ziekten, die betrokkenen van heinde en verre samenbrengt: van patiënten tot artsen, overheid, academie, verzekeraars en bedrijven. De conferentie stond in het teken van strategieën voor samenwerking. Een mooie gelegenheid om de PEG – als handvat voor samenwerking tussen bedrijven en patiënten – te lanceren en met een interactieve poster aan de congresbezoekers te presenteren.

In het HollandBIO programma “Sneller en Beter van lab naar patiënt” werkt HollandBIO met koplopers uit de biotech aan oplossingen die het ontwikkeltraject van geneesmiddelen verbeteren en versnellen. Annemiek Verkamman, directeur van HollandBIO, licht toe: “Biotech levert revolutionaire nieuwe geneesmiddelen en therapieën. Maar er staan nog te veel patiënten met lege handen. Om ook die patiënten hoop te geven op een effectieve behandeling op maat, moeten we dringend werk maken van oplossingen die gezondheidswinst, innovatie en betaalbaarheid verenigen.”

Een centrale rol voor het patiëntenperspectief in elke fase van geneesmiddelenontwikkeling is zo’n oplossing. Een patiënt weet immers als geen ander wat een geneesmiddel moet doen om het verschil te maken. Bovendien kan de patiënt als ervaringsdeskundige het ontwikkeltraject een boost geven, zowel qua kwaliteit als voortgang. Ook vanuit zakelijk oogpunt is het daarmee logisch om de patiënt in elke fase van de geneesmiddelontwikkeling centraal te stellen.

“Het klinkt zo makkelijk: patiënten betrekken bij het doen van geneesmiddelonderzoek”, zegt Philipp von Gallwitz, medeoprichter en managing partner bij admedicum® Business for Patients. “Maar de praktijk blijkt weerbarstig. Want hoe organiseer je dat eigenlijk? De ervaring leert dat lang niet elke organisatie hier even bedreven in is.” Om bedrijven de helpende hand te bieden, ontwikkelden HollandBIO en admedicum samen de PEG. De PEG is een online handboek, dat goede én minder goede voorbeelden van patiëntbetrokkenheid in de praktijk uitlicht. Daarnaast wijst het de weg in het oerwoud aan informatie over dit onderwerp. Groen licht medicijn Kiadis nabij

Topman Arthur Lahr van Kiadis Pharma heeft er vertrouwen in dat zijn medicijn Atir101 volgend jaar op de Europese markt wordt toegelaten.

Acute leukemie en andere vormen van bloedkanker worden veelal met chemotherapie behandeld. Dit werkt echter niet voor alle patiënten. Voor hen kan een allogene stamceltransplantatie uitkomst bieden. Hoewel alleen ons land al ruim tweehonderdduizend donors telt, is het niet eenvoudig om een goede match te vinden. Ook tussen familieleden zoals ouder en kind is gewoonlijk hooguit sprake van een halve match. Juist bij hen zorgt het gebruik van Atir101 ervoor dat een transplantatie veel effectiever en veiliger kan plaatsvinden, zo wijzen de door Kiadis uitgevoerde patiëntenstudies uit.

Het wachten is nu op toestemming van de EMA en Lahr acht de kans groot dat deze er in het vierde kwartaal daadwerkelijk komt. „In dit stadium is de kans gemiddeld genomen 80%. En wij denken hier nog wat boven te zitten, gezien de aard van de vragen die de EMA ons heeft gestuurd.”

Terwijl kleine biotechnologiebedrijven de marketing en verkoop van hun medicijn veelal uitbesteden aan een geneesmiddelenfabrikant, zet Kiadis zelf een commerciële infrastructuur op. „De transplantaties vinden vrij geconcentreerd plaats, voor wat betreft de vijf grootste EU-landen in 70 klinieken. Met enkele daarvan werken we nu al samen. De productie gebeurt op dit moment in Duitsland maar in de toekomst zal dit ook worden gedaan op onze locatie vlakbij het AMC in Amsterdam”, meldt Lahr.

Concurrentie ondervindt Kiadis vooral van het Amerikaanse Baltimore-protocol, waarbij chemotherapie na de transplantatie van een voor de helft gematchte familiedonor plaatsvindt. „Afgaande op de literatuur en onze fase 2-studie lijkt de overlevingskans vergelijkbaar, maar bij ons product lijkt het risico kleiner dat de ziekte weer terugkomt. Er is ook minder risico dat het immuunsysteem van de donor de patiënt aanvalt”, stelt Lahr.

Terwijl de topman erop vertrouwt dat Kiadis in het derde kwartaal van 2019 met de verkoop van Atir101 in Europa kan beginnen, laat de marktintroductie in de VS langer op zich wachten, aangezien voor de VS nog een zogenaamde fase 3-studie nodig is. „Ik geef geen prognose over de timing. Wel is het zo dat van de 50.000 patiënten in beide continenten die jaarlijks baat zouden kunnen hebben bij ons product, de meerderheid in Europa woont. De VS hebben we ook niet per se nodig om winstgevend te kunnen worden.”

‘Betrek patiëntervaring bij keuze vergoeding medicijn’

Gegevens uit de dagelijkse klinische praktijk en ervaringen van patiënten moeten meer invloed krijgen op de keuze om geneesmiddelen al dan niet te vergoeden. Dat betoogt farmaceutisch onderzoeker Amr Makady in zijn proefschrift.

Beleidsmakers in de gezondheidszorg maken momenteel meestal gebruik van de uitkomsten van health technology assessments (HTA). Dat zijn gerandomiseerde klinische studies, met vooraf geselecteerde, homogene patiëntgroepen. Voor een goed beeld van de werking van een medicijn zouden beleidsmakers als het aan Makady ligt echter ook toegang moeten hebben tot zogenoemde ‘real-world data’.

Die informatie is te verzamelen via diverse bronnen waar patiënten zelf hun ervaringen met een medicijn delen, zoals patiëntregistraties en zelfs social media. Makady: “Uit reacties op een patiëntforum kan bijvoorbeeld blijken dat een medicijn in de praktijk mogelijk minder goed werkt dan eerder was geconcludeerd uit de klinische studies. Of juist dat een medicijn wel vervelende bijwerkingen heeft, maar dat patiënten die bijwerkingen graag voor lief nemen.”

Zorginstituut Nederland, waar Makady werkt, maakt inschattingen van de kosteneffectiviteit van geneesmiddelen over lange periodes, bijvoorbeeld over dertig jaar. De klinische studies waarop het instituut haar conclusies baseert, biedt vaak inzicht over veel kortere periodes. Actuele, real-world data kan volgens Makady mogelijk veel relevantere en misschien wel net zulke betrouwbare inzichten bieden over de ervaringen van grote groepen mensen met een bepaald medicijn.

Voorwaarden

Er moeten wel voorwaarden gelden voor het gebruik van dit type data, aldus de onderzoeker. Zo moeten de diverse betrokken partijen het eens zijn over de vraag wat real-world data eigenlijk precies inhoudt. “Verder is het belangrijk dat partijen afstemmen hoe deze data geanalyseerd en geïnterpreteerd dient te worden. Het is ook vanzelfsprekend dat we moeten nadenken over de privacy van personen bij dit type data. Sowieso zou overigens alleen geanonimiseerde informatie van patiënten gebruikt mogen worden.”

Johnson & Johnson’s Darzalex snags landmark myeloma approval in first-line patients

Johnson & Johnson’s Darzalex just vaulted ahead of its myeloma competitors—and it has plenty of them.

Monday, the FDA greenlighted the drug—in combination with Takeda’s Velcade and prednisone—in first-line multiple myeloma patients who are transplant ineligible, making it the first monoclonal antibody to ring up an approval in newly diagnosed patients.

The go-ahead is no surprise, considering the stellar data J&J unveiled at December’s American Society of Hematology annual meeting. In a trial of front-line patients, at a median follow-up of 16.5 months, the Darzalex regimen cut the risk of disease progression or death by 50%.

Darzalex has come a long way since its first approval back in late 2015, which came the same month Bristol-Myers Squibb’s Empliciti and Takeda’s Ninlaro picked up their own FDA blessings. The J&J product was initially cleared only for patients who had tried three prior treatments, and it snagged its second-line OK a year later.

Meanwhile, the newest approval marks Darzalex’s fifth in myeloma—and the way J&J’s executives see it, there are plenty more to come, including in the front-line setting.

“What we’ve seen is that the drug seems to benefit with any regimens—with other IMiD-containing regimens, with proteasome regimens … so our general view is that this is a mechanism that can improve on [results from] any established regimen,” Peter Lebowitz, Janssen’s global therapeutic area head of oncology, said in an interview at ASH.

The company’s Janssen unit is currently trialing the hot-shot drug in other front-line studies, one of which is examining Darzalex in combination with an IMid therapy and a proteasome inhibitor. “We want it to be fairly universal that you use the drug with whatever regimen you end up choosing,” Lebowitz said.

Intravacc and Provention Bio collaborate on Coxsackievirus B Vaccine

Provention, a clinical stage biopharmaceutical company dedicated to sourcing, developing and commercializing novel therapeutics aimed at intercepting and preventing immune-mediated diseases, announced that it has selected Netherlands-based Intravacc to lead product development and manufacturing of clinical trial material for Provention’s Coxsackievirus B (CVB) vaccine (PRV-101).

France-based L2D Services SARL/Leads-To-Development (L2D) is providing expert development, regulatory and project management support to ensure rapid progress of PRV-101 into clinical development. CVB, an enterovirus, has been identified as a possible common trigger for type 1 diabetes (T1D) onset. Vaccination of at-risk populations against CVB infection may lead to the prevention or delay in onset of T1D. The CVB vaccine was developed initially by Vactech Ltd. of Finland and licensed to Provention in April, 2017.

Provention is planning to initiate first in-human clinical trials in 2020. Enteroviruses are responsible for an estimated 30 million infections annually in the US. CVB contributes significantly to enteroviral healthcare costs, since it can result in hospitalization and severe morbidity. Usually asymptomatic or presenting as cold-like symptoms, fever, rash, hand-foot-mouth disease, and/or mild gastrointestinal distress, CVB infections can occasionally cause more serious and sometimes lifethreatening illnesses including pericarditis, myocarditis, meningitis and pancreatitis.

Provention, in partnership with Intravacc, Vactech and L2D is now developing its CVB vaccine for large scale manufacture of clinical trial material in accordance with current Good Manufacturing Practices (cGMP). Intravacc was chosen as Provention’s manufacturing partner based on its expertise and excellent track in enterovirus vaccine development, including IPV, sIPV, EV-71, and CV-A16, as well as Intravacc’s state-of-the-art vaccine cGMP production facilities. PRV-101 will be developed using Intravacc’s proven and well-established platform technology, including certain proprietary cell lines and production processes.

Europa pleit voor sterkere samenwerking op vaccineerbare aandoeningen

De Europese Commissie (EC) publiceert een voorstel, een zogenaamde Council Recommendation, om samenwerking rond vaccineerbare aandoeningen te versterken. Dit voorstel volgt kort op de oproep van het Europees Parlement voor een hogere vaccinatiegraad. EU-wijde actie is hard nodig, vindt ook de EC: “omdat zwakte van één land bij vaccineren, de gezondheid en veiligheid van alle burgers in Europa op het spel zet”. In het voorstel daagt de EC zowel zichzelf als individuele lidstaten uit om zich in te zetten voor een hoge vaccinatiegraad, waarborging van toegang en overbrugging van verschillen en gaten in nationale vaccinatieschema’s. HollandBIO juicht dit voorstel toe: want samen staan we sterk in de strijd tegen infectieziekten!

In de EU is elk land baas over eigen vaccinatieprogramma. Hierdoor bestaan verschillen in de nationale vaccinatieplannen, -organisatie en -uitvoering. Maar ondanks deze verschillen, vinden de lidstaten elkaar in diverse gedeelde uitdagingen: een dalende vaccinatiegraad, tekorten en een groeiende groep vaccintwijfelaars. Om deze uitdagingen het hoofd te bieden, klopten meerdere EU lidstaten bij de EC aan. Dat leidde tot een voorstel met aanbevelingen om de samenwerking rond vaccineerbare aandoeningen te versterken.

In het voorstel nodigt de EC lidstaten uit om vaccinaties in verschillende omgevingen aan te bieden, bijvoorbeeld in apotheken, om processen te versimpelen en waar nodig specifieke, kwetsbare groepen te benaderen. Ook zijn de lidstaten verantwoordelijk voor heldere voorlichting, via goed getrainde zorgmedewerkers.

De EC en lidstaten worden tevens aangespoord om samen een “European Vaccine Information system” en een “virtual data warehouse” te ontwikkelen. Het eerste systeem zal up-to-date informatie over vaccins op Europees niveau delen, om bijvoorbeeld op termijn stakeholders samen te brengen om richtlijnen te vormen voor een Europees “basisvaccinatieschema”. Het tweede systeem zal vaccinbehoeften en voorraden in kaart brengen. In nauwe samenwerking met de industrie moet dit de productiecapaciteit verbeteren en tekorten voorkomen. Het virtueel data-warehouse kan in geval van nood voor uitwisseling van vaccins zorgen of aanspraak op een –  nog te vormen – Europese voorraad regelen.

Tot slot draagt de EC zelf ook een steentje bij. De commissie heeft de intentie om een gemeenschappelijke EU vaccinatiekaart te vormen, om het vertrouwen van vaccinatie niet alleen regelmatig te monitoren maar ook te publiceren, en om een internationale Coalitie voor Vaccinatie te vormen die vaccinatie en samenwerking op dit vlak promoot.

HollandBIO kijkt reikhalzend uit wat Nederland gaat doen met de aanbevelingen. In de Joint Action on Vaccination, waarbij Nederland nauw betrokken is, wordt in elk geval hard gewerkt aan een aantal van deze oplossingen. En dat is belangrijk, want voorkomen is immers beter dan genezen!

CHMP Backs Expanded Use For Amgen’s Prolia To Patients With Glucocorticoid-Induced Osteoporosis

Amgen announced that the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) has adopted a positive opinion for the marketing authorization of Prolia for the treatment of bone loss associated with long-term systemic glucocorticoid therapy in adult patients at increased risk of fracture.

“Today’s positive opinion by the CHMP is an important step for Prolia in helping patients suffering from bone loss associated with systemic glucocorticoid therapy,” said Sean E. Harper, M.D., executive vice president of Research and Development at Amgen. “Chronic use of oral glucocorticoids has been associated with an increase in spine and hip fractures,1 and, if approved, an expanded use of Prolia will provide patients and physicians across much of Europe with a new treatment option for this serious condition.”

Glucocorticoid medications, which are used to treat many inflammatory conditions such as chronic obstructive pulmonary disorder (COPD), asthma, multiple sclerosis and rheumatoid arthritis, can cause significant side effects, including bone loss.

The CHMP recommendation is supported by a Phase 3 randomized, double-blind, double-dummy, active-controlled study evaluating the safety and efficacy of Prolia compared with risedronate in patients receiving glucocorticoid treatment.4 The study included two patient groups: those on sustained glucocorticoid therapy and those newly initiating glucocorticoid therapy. The study met the primary endpoint (percent change from baseline in lumbar spine bone mass density [BMD] at 12 months, assessing non-inferiority) and all secondary endpoints (the percent changes from baseline in lumbar spine and total hip BMD at 12 and 24 months, assessing superiority). Study results showed that, in patients on sustained glucocorticoid therapy, Prolia treatment led to greater gains in BMD, compared with risedronate, both at the lumbar spine (4.4 percent versus 2.3 percent, respectively) and total hip (2.1 percent versus 0.6 percent, respectively). Similarly, in patients newly initiating glucocorticoid therapy, Prolia treatment led to greater increases in BMD, compared with risedronate, both at the lumbar spine (3.8 percent versus 0.8 percent, respectively) and total hip (1.7 percent versus 0.2 percent, respectively).

Adverse events and serious adverse events were similar between treatment groups and consistent with the known safety profile of Prolia. No serious adverse events were reported with a subject incidence of two percent or greater in either treatment group.

The CHMP positive opinion will now be reviewed by the European Commission (EC), which has the authority to approve medicines for the European Union (EU). Norway, Iceland and Liechtenstein, as members of the European Economic Area (EEA), will take corresponding decisions based on the decision of the EC.

The U.S. Food and Drug Administration (FDA) is currently reviewing a supplemental Biologics License Application for this expanded indication and has set a Prescription Drug User Fee Act (PDUFA) action date of May 28, 2018.

pLADD Therapy Found to Induce Both Innate and Adaptive Immunity

Aduro Biotech presented preliminary observations from a case study of a patient with metastatic colorectal cancer treated in Aduro’s ongoing Phase 1 study of its personalized neoantigen-based immunotherapy (pLADD). This Phase 1 proof-of-concept study is designed to evaluate the safety and tolerability of pLADD immunotherapy in adults with metastatic colorectal cancer that is microsatellite stable (MSS). Data were presented at the European Neoantigen Summit held in Amsterdam (April 24-26, 2018).

The immunological data presented demonstrated that neoantigens isolated and sequenced from the patient’s tissue samples, and engineered into a personalized immunotherapy, induced neoantigen-specific CD8+ T cells undetectable before pLADD treatment started. In addition to adaptive immunity, pLADD induced an innate response exemplified by gamma delta T cells, also thought to be important for successful immunotherapy. The patient’s neoantigens were selected using state-of-the-art algorithm identification technology developed by Aduro’scollaborator, Hanlee Ji, M.D., associate professor of medicine at the Stanford University School of Medicine.

“Although early, the immunological data obtained from this case study is encouraging, as it indicates that our pLADDimmunotherapy has the potential to induce a sustained, antigen-specific effect on the immune system,” said Andrea van Elsas, Ph.D., chief scientific officer of Aduro. “We believe our pLADD approach could offer a differentiated treatment option to patients with MSS colorectal cancer, who represent the vast majority of the colorectal cancer patient population and who have not been responsive to immune checkpoint inhibitors. In addition, these preliminary observations support our plan to combine pLADD with checkpoint inhibitors, which we believe could enhance the overall response in this patient setting. We look forward to reporting additional immunological data from this Phase 1 trial before the end of 2018.”

Preclinical data presented showed that mouse pLADD strains targeting tumor-specific neoepitopes induced a robust immune response, including induction of cytokines, chemokines, and antigen-specific CD8+ T cells.  In preclinical models of pLADD, remodeling of the tumor microenvironment with an increase in the CD8:Treg ratio was observed. The combination of pLADD with an anti-PD-1 antibody led to a sustained immune response and significantly improved efficacy in these mouse tumor models.
