Alnylam’s GIVLAARI receives FDA approval

Alnylam has received FDA approval for GIVLAARI™ (givosiran) for the treatment of adults with acute hepatic porphyria (AHP). AHP is a family of ultra-rare, genetic diseases characterized by debilitating, potentially life-threatening attacks and, for some patients, chronic manifestations that negatively impact daily functioning and quality of life. Long-term complications of AHP can include chronic neuropathic pain, hypertension, chronic kidney disease and liver disease. GIVLAARI was shown to significantly reduce the rate of porphyria attacks that required hospitalizations, urgent healthcare visits or IV hemin administration at home.

 “We believe the approval of GIVLAARI represents a landmark event for the advancement of precision genetic medicines, providing new hope for patients and their caregivers living with the debilitating manifestations of AHP and unpredictable nature of AHP attacks, as well as for the doctors who diagnose and treat these patients. We are grateful to the investigators, patients and families who have helped make this new treatment option a reality for the AHP community. The FDA approval of GIVLAARI was received in less than four months after acceptance of the NDA, and was based on positive results from the ENVISION Phase 3 study, a randomized, double-blind, placebo-controlled, multinational study of 94 patients with AHP, at 36 study sites in 18 countries – the largest ever interventional study conducted in AHP. In ENVISION, AHP patients on GIVLAARI experienced 70% (95% CI: 60%, 80%) fewer porphyria attacks compared to placebo. GIVLAARI also resulted in a similar reduction in intravenous hemin use, as well as reductions in urinary aminolevulinic acid (ALA), and urinary porphobilinogen (PBG).

In the pivotal ENVISION study, the most common adverse reactions (reported in at least 20% of patients) with GIVLAARI were nausea (27%) and injection site reactions (25%). Other adverse reactions seen in patients treated with GIVLAARI (occurring over 5% more frequently than placebo) include rash, serum creatinine increase, transaminase elevations and fatigue. As previously reported, one patient in the GIVLAARI clinical development program experienced an anaphylactic reaction which resolved with medical management.

“Adults with AHP now have a new treatment option that has demonstrated the ability to reduce the frequency of porphyria attacks by specifically addressing factors associated with attacks and other disease manifestations of AHP,” said Manisha Balwanii, M.D., M.S, Associate Professor of the Department of Genetics and Genomic Sciences and Department of Medicine at the Icahn School of Medicine at Mount Sinai and principal investigator of the ENVISION study. “With the approval of GIVLAARI, and based on the efficacy data from the ENVISION study, I hope to see my patients and those across the country be able to live more normal lives with fewer porphyria attacks.”

In August, Alnylam announced a U.S. gastrointestinal (GI) disease education and promotional agreement for GIVLAARI with Ironwood Pharmaceuticals, Inc., a GI healthcare company. Under the agreement, Alnylam will leverage Ironwood’s leading capabilities in GI to help raise awareness of AHP among gastroenterologists and other healthcare practitioners in the U.S. Ironwood will also participate in GIVLAARI promotional efforts, augmenting Alnylam’s broader commercialization activities.

GIVLAARI was reviewed by the FDA under Priority Review and had previously been granted Breakthrough Therapy and Orphan Drug Designations in the U.S. GIVLAARI is currently being reviewed under accelerated assessment by the European Medicines Agency (EMA) for the treatment of patients with AHP, after receiving Priority Medicines (PRIME) Designation and Orphan Drug Designation from the EMA.

Novo Nordisk eerste deelnemer Pilot Parallelle Procedures CBG-ZIN

Novo Nordisk levert de eerste kandidaat aan voor de pilot Parallelle Procedures CBG-ZIN. Het gaat om een aanvraag voor een orale toedieningsvorm van semaglutide, een medicijn bestemd voor patiënten met type 2 diabetes mellitus. In de pilot slaan CBG en ZIN de handen ineen om de tijd van lab naar patiënt te verkorten, door eerder te starten met het nationale vergoedingstraject en dit (deels) parallel te laten lopen met het Europese registratietraject.

HollandBIO is erg enthousiast over de pilot, omdat deze naadloos aansluit bij het HollandBIO programma Sneller & beter van lab naar patiënt. Stijging uitgaven weesgeneesmiddelen valt vies tegen

Op 22 november publiceerde het Zorginstituut de Monitor ‘Weesgeneesmiddelen in de praktijk’. De kernboodschap: de uitgaven aan weesgeneesmiddelen stijgen gemiddeld met 9% per jaar. In 2017 werd er 4% meer uitgegeven aan weesgeneesmiddelen dan in 2016. HollandBIO vindt dat groeicijfer ronduit teleurstellend. Er moet echt een tandje bij.

De cijfers uit de Monitor ‘Weesgeneesmiddelen in de praktijk’ zijn klip en klaar: het basispakket bevatte in 2017 49 weesgeneesmiddelen, waarmee in totaal 6.900 patiënten van een behandeling voorzien werden. Hoewel de monitor het aantal indicaties onvermeld laat, is het evident dat er voor het leeuwendeel van de 6.000 zeldzame ziekten nog geen behandeling beschikbaar is. De Horizonscan Geneesmiddelen verwacht de komende jaren gelukkig 57 nieuwe weesgeneesmiddelen of indicatie-uitbreidingen. Het is te hopen dat die de patiënt wat sneller bereiken dan het huidige tempo van zo’n 4 nieuwe weesgeneesmiddelen per jaar. Want als je lijdt aan een zeldzame, vaak progressieve, ziekte, dan is elke dag wachten op een behandeling er één teveel. HollandBIO kijkt dan ook reikhalzend uit naar double-digit groeicijfers volgend jaar!

In aanloop naar de publicatie van de definitieve monitor weesgeneesmiddelen vroeg HollandBIO onder meer aandacht voor gezondheidswinst, snelle toegankelijkheid en een bijbehorend, passend, beoordelingskader voor weesgeneesmiddelen. Onze inbreng en de reactie daarop van het Zorginstituut, is opgenomen in het rapport (pagina 115 en 131).

Effectief vaccinbeleid centraal in Europees beleidsmagazine

Staatssecretaris Blokhuis is niet de enige die zoveel mogelijk gezondheidswinst met vaccinatie wil verzilveren. Ook elders in Europa houdt vaccinatie de gemoederen bezig.  Het magazine voor Europese instanties en beleidsmakers, “The European Files”,  wijdt daarom een special aan vaccinaties. Effectief beleid, zowel op nationaal als Europees niveau staat hierin centraal. Diverse partijen, waaronder Europese en nationale Ministers, EMA, ECDC, WHO, IMI, EPF, CPME, PGEU en industrie geven een kijkje in eigen keuken, of reflecteren op mogelijke verbeteringen op Europees niveau. Een absolute leestip als het vaccinatiedossier je na aan het hart gaat!

MSD verdubbelt biotechproductie in Oss

MSD, het grootste geneesmiddelenbedrijf in Nederland, verdubbelt de komende jaren haar capaciteit voor biotechproductie in Oss. De uitbreiding is nodig vanwege de wereldwijd groeiende vraag naar behandelingen tegen diverse soorten kanker. Met de vergroting van de capaciteit en de nieuwe faciliteiten kan MSD vanuit Oss geneesmiddelen leveren aan de rest van de wereld.

Nederlands tintje aan wereldwijde kankerbestrijding

Kanker is één van de grootste uitdagingen van onze tijd. In Nederland krijgen elk jaar ongeveer 116.000 mensen kanker (1). Wereldwijd overlijden ongeveer 1 op de 6 mensen aan de ziekte (2). Er zijn verschillende soorten behandelingen voor verschillende vormen van kanker, zoals doelgerichte therapie, chemotherapie en immuuntherapie. MSD richt zich vooral op immuuntherapie. MSD Nederland is – vooral in Oss – nauw betrokken geweest bij de ontwikkeling van immuuntherapie en speelt daar dus nog steeds een belangrijke rol in met biotechnologische productie.

16 miljard dollar

De investeringen in Oss maken deel uit van een meerjarenprogramma waarmee MSD 16 miljard dollar investeert om productiecapaciteit en -faciliteiten over de hele wereld uit te breiden. Binnen Nederland wordt op dit moment op de locaties van MSD in Haarlem, in Oss en in Boxmeer (diergezondheid) volop gebouwd.

Passie en trots

“Met nieuwe high tech faciliteiten en meer productiecapaciteit gaat Nederland een belangrijke bijdrage leveren aan de bestrijding van kanker. Met deze uitbreiding tonen we onze betrokkenheid bij de behandeling van kanker en dat doen we uiteindelijk voor al die patiënten”, vertelt John Glavas, directeur van MSD Oss Biotech. “Ik ben trots op onze medewerkers die elke dag met passie en trots aan deze behandelingen werken.” De uitbreiding is daarnaast goed voor de BV Nederland. MSD heeft momenteel 6.000 medewerkers in Nederland en is actief op het gebied van ontwikkeling, klinisch onderzoek, productie, verpakking en distributie.

Novo Nordisk and UNICEF partner to prevent childhood overweight and obesity

On 20 November 2019, for World Children’s Day, Novo Nordisk and UNICEF announced an ambitious and collaborative multi-million-dollar global partnership to address the global childhood obesity crisis. 

UNICEF is broadening its programmatic focus on the prevention of childhood overweight and obesity in children and adolescents and early life prevention of non-communicable diseases (NCDs). Novo Nordisk is a global healthcare company with an ambition to defeat diabetes and other serious chronic diseases, recognizing prevention as a key building block to help build healthier environments to enable improved health. 

UNICEF and Novo Nordisk aim to combine their expertise, reach and convening power to make a significant contribution to the global agenda to prevent childhood overweight and obesity, reaching more than half a million children in Latin America by 2023. The long-term goal is to support governments and decision makers to act and change public policy on this issue. 

Child overweight has far reaching consequences

This partnership responds to UNICEF’s 2019 State of the World’s Children Report, focusing on children, food and nutrition, which provides a stark warning that childhood overweight and obesity is increasing across all continents. Today, 40 million children under the age of 5 are overweight, while the proportion of children and adolescents aged 5 -19 years old who are overweight has nearly doubled in the past 15 years. 

Children affected by overweight and obesity may face challenges in thriving and reaching their full potential. Being overweight can contribute to stigmatization, poor socialization and emotional difficulties and in some cases reduced educational attainment. The Report of the Commission on Ending Childhood Obesity (WHO 2016) also states that child overweight can lead to a higher risk of developing health problems, including musculoskeletal, orthopedic complications and chronic non-communicable diseases (NCDs). 

A complex issue where early prevention is key

The drivers of the recent increase in overweight are multiple. Changes to the modern food supply appear to be one of the dominant drivers of weight gain. This includes inappropriate marketing and advertising, the abundance of ultra-processed foods in cities but also in remote areas and increasing access to fast food and highly sweetened beverages. Other factors contributing to increasing rates of overweight include; poverty and urbanization, as well as declines in physical activity, active transport and active play time due to insufficient access to safe exercise spaces in cities and increased screen time. 

The most efficient solution is to prevent overweight and obesity from developing at an early age.

Novo Nordisk comes with a significant financial and technical commitment to support UNICEF’s efforts on prevention of childhood overweight and obesity. Kansen voor slimmer meten: internationale richtlijn medicijnonderzoek aangepast

Door betere inzet van in vitro-testen kunnen wereldwijd veel dierproeven worden voorkomen. Dit kondigt het College ter Beoordeling van Geneesmiddelen deze week aan. De reden hiervoor is dat de internationale richtlijn voor het testen van de schadelijkheid van medicijnen tijdens de zwangerschap voor het eerst in 25 jaar wordt aangepast.

Op dit moment moeten geneesmiddelenbedrijven nog in twee verschillende diermodellen (bv. konijn en rat) aantonen dat een medicijn niet schadelijk is voor de foetus. Volgens de nieuwe richtlijn zijn tijdens fase 1 en 2 van het geneesmiddelenontwikkelingstraject straks minder grote studiepopulaties nodig en mag een in vitro-test één van de twee diermodellen vervangen. Dit voorkomt onnodige dierproeven voor medicijnen die in veel gevallen de eindstreep niet halen. Daarnaast zullen in vitro-testen in bepaalde gevallen afdoende zijn, bijvoorbeeld als het gaat om een stof uit een medicijngroep waarvan bekend is dat ze schadelijk zijn tijdens zwangerschap.

Hoewel de aanpassing van de internationale richtlijn een mooie eerste stap is op weg naar snellere en betere geneesmiddelenontwikkeling, zijn we er nog lang niet. Zo blijft voor fase 3 onderzoek de dubbele dierproef vereist. Waarom? Wat staat er nog in de weg om de transitie naar het proefdiervrij ontwikkelen van nieuwe geneesmiddelen mogelijk te maken? Steeds meer bedrijven in onze achterban ontwikkelen innovatieve technologieën die hier een rol in kunnen spelen, zoals organoids of organs-on-a-chip. HollandBIO inventariseert momenteel de knelpunten op weg naar snellere en betere inzet van dit soort “slimmer meten” technologieën.

Merck benoemt Saraswati Khan als GM voor NL Healthcare Business

Saraswati Khan is met ingang van 1 november benoemd als General Manager Biopharma (Healthcare) voor Merck Nederland. Haar standplaats wordt de Amsterdamse vestiging van de gezondheidsdivisie van het bedrijf (Merck BV).

Saraswati begon haar loopbaan bij Merck in 2009 in India en heeft meer dan 25 jaar ervaring op de gezondheidszorgmarkt. Aanvankelijk was ze werkzaam in sales, daarna vervulde ze een reeks van marketing- en leidinggevende functies. Binnen Merck Healthcare heeft ze met succes als Business Unit Head leiding gegeven aan teams, eerst in India en vervolgens in Brazilië, in alle franchisen (algemene geneeskunde, oncologie, vruchtbaarheid, neurologie en immunologie). Ze heeft een sterke staat van dienst als het gaat om het leveren van succesvolle bedrijfsresultaten. 

“Saraswati laat zien over flexibiliteit en cultureel aanpassingsvermogen in grote, diverse en complexe markten te beschikken. Ze is in staat om capabele en betrokken teams op te bouwen, en weet tegelijkertijd winstgevende groei te waarborgen”, aldus Renée C. WHO prequalifies Ebola vaccine, paving the way for its use in high-risk countries

The World Health Organization (WHO) today prequalified an Ebola vaccine for the first time, a critical step that will help speed up its licensing, access and roll-out in countries most at risk of Ebola outbreaks. This is the fastest vaccine prequalification process ever conducted by WHO.

Prequalification means that the vaccine meets WHO standards for quality, safety and efficacy. United Nations agencies and Gavi, the Vaccine Alliance, can procure the vaccine for at-risk countries based on this WHO recommendation.

“This is a historic step towards ensuring the people who most need it are able to access this life-saving vaccine,” said Dr Tedros Adhanom Ghebreyesus, WHO Director-General. “Five years ago, we had no vaccine and no therapeutics for Ebola. With a prequalified vaccine and experimental therapeutics, Ebola is now preventable and treatable.”

The injectable Ebola vaccine, Ervebo, is manufactured by Merck (known as MSD outside the US and Canada). It has been shown to be effective in protecting people from the Ebola Zaire virus and is recommended by the WHO Strategic Advisory Group of Experts (SAGE) for vaccines as part of a broader set of Ebola response tools. The decision is a step towards greater availability of the vaccine in the future, though licensed doses will only be available mid-2020.

This announcement comes less than 48 hours after the European Commission decision to grant a conditional marketing authorization for the vaccine, following the recommendation from the European Medicines Agency (EMA).

Due to the urgent public health need for a prequalified Ebola vaccine, WHO accelerated prequalification by reviewing safety and efficacy data as the information became available. Representatives from the prequalification team participated in the EMA evaluation process to address programmatic suitability for at-risk countries in Africa.

“The development, study, and rapid prequalification of this vaccine show what the global community can do when we prioritize the health needs of vulnerable people,” said Dr Tedros.

WHO is also facilitating licensing of the vaccine for use in countries at risk of Ebola outbreaks, based on the reviews and positive outcome by the EMA. WHO, with the support of EMA, has worked closely with many African regulators who have indicated they will quickly license the vaccine following the WHO recommendation.
