Orchard Therapeutics Receives Positive CHMP Opinion for Libmeldy™ for the Treatment of Early-Onset Metachromatic Leukodystrophy (MLD)

Orchard Therapeutics (Nasdaq: ORTX), a global gene therapy leader, announced that the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) has adopted a positive opinion recommending full, or standard, marketing authorization for Libmeldy (cryopreserved autologous CD34+ cells encoding the arylsulfatase-A, or ARSA, gene), an investigational gene therapy for the treatment of metachromatic leukodystrophy (MLD), characterized by biallelic mutations in the ARSA gene leading to a reduction of the ARSA enzymatic activity in children with i) late infantile or early juvenile forms, without clinical manifestations of the disease, or ii) the early juvenile form, with early clinical manifestations of the disease, who still have the ability to walk independently and before the onset of cognitive decline.

The CHMP’s positive opinion will now be reviewed by the European Commission (EC), which has the authority to grant marketing authorization for Libmeldy in the European Union (EU). A final decision by the EC for Libmeldy is anticipated before the end of 2020. If approved, Libmeldy would be the first commercial therapy and first gene therapy for eligible patients with early-onset MLD.

MLD is a very rare, severe genetic condition caused by mutations in the ARSA gene which lead to neurological damage and developmental regression. In its most severe and common forms, young children rapidly lose the ability to walk, talk and interact with the world around them. A majority of these patients pass away in childhood, with palliative care often as their only option.

“Today’s positive CHMP opinion for marketing authorization of Libmeldy is a remarkable achievement that we share with the MLD community, as it brings us closer to delivering a one-time, potentially transformative therapy for eligible children suffering from this devastating disease,” said Bobby Gaspar, M.D., Ph.D., chief executive officer, Orchard Therapeutics. “Data from the Libmeldy clinical program have demonstrated the potential for long-term positive effects on cognitive development and maintenance of motor function, translating to individual preservation of motor milestones such as the ability to sit, stand and/or walk without support, as well as attainment of cognitive skills like social interactions and school attendance, at ages at which untreated patients show severe motor and cognitive impairments.”

Libmeldy is designed as a one-time gene therapy, developed in partnership with the San Raffaele-Telethon Institute for Gene Therapy (SR-Tiget) in Milan, Italy, in which the patient’s own hematopoietic stem cells (HSCs) are selected, and functional copies of the ARSA gene are inserted into the genome of the HSCs using a lentiviral vector before these genetically modified cells are infused back into the patient. The ability of the gene-corrected HSCs to migrate across the blood-brain barrier into the brain, engraft, and express the functional enzyme has the potential to persistently correct the underlying genetic condition with a single treatment.

These results also further illustrate our view that the HSC gene therapy approach has the potential to deliver transformative effects in other storage diseases as well, especially when the cells are designed to overexpress the functional enzyme and provide an enhanced supply of it to the affected tissues.”

Orca Therapeutics Announces Positive DSMB Review in Phase 1/2a Study of ORCA-010in Treatment-Naïve Prostate Cancer Patients

Orca Therapeutics BV, a clinical-stage biopharmaceutical company developing oncolytic viruses to treat solid tumors, is pleased to announcethat, upon review of all safety data from the fully enrolled, low-dose patient cohort of the ongoing Phase 1/2a clinical study of ORCA-010in treatment-naïve patients with localized prostate cancer, the independent Data and Safety Monitoring Board (DSMB) unanimously recommended the continuation of the study without modification.

Following this recommendation, Orca Therapeutics has initiated enrollment in the intermediate-dose arm of the dose-escalation phase of this study. Two additional, pre-specified DSMB reviews will occur after the completion of enrollment in the intermediate-dose study arm and the high-dose study arm, respectively.

At an earlier stage,ORCA-010 demonstrated strong single-agent antitumor activity in the first patient treated with the lowest dose in this study. MRI data is impressive reflecting significant decrease in prostate volume returning to normal sizeas well asless tumor tissuesurface area. Further of highlighted interest,the researchers reported an ongoing decrease in PSA.

“We are encouraged by the safety profile and preliminary efficacy of ORCA-010so far and are excited to continue enrollment in the intermediate dose cohort of this Phase 1/2a study, our first clinical study with ORCA-010,” said Kees Groen, Orca’s CEO.

Orca’stwo-part Phase 1/2a study is a multi-center, open-label, dose-escalation study of ORCA-010in treatment-naïve patients with localized prostate cancer:

Part A: Dose-escalation phase to define the safety, tolerability, and optimal dose level of ORCA-010 in treatment-naïve patients with localized prostate cancer. This phase is expected to enroll up to 12patients in three dose cohorts.

Part B: A subsequent extension of the optimal dose level, as defined in the dose escalation phase, and given as two doses with 2 weeks interval. This phase is expected to enroll 12additional patients.

Tweeëntwintig jaar engelengeduld

“Na 22 jaar geduldig onderzoek zet biotechbedrijf Galapagos eerste medicijn op de markt: ‘een enorme doorbraak’”, zo kopte de Volkskrant in een interview met topman Onno van de Stolpe. Recent heeft Galapagos namelijk een handelsvergunning verkregen in de Europese Unie en Japan voor Jyseleca® (filgotinib) voor de behandeling van volwassenen met matige tot ernstige reumatoïde artritis.

Het is iets om bij stil te staan, want zo vaak gebeurt het niet dat een bedrijf op Nederlandse bodem uitgroeit van start-up tot innovatieve biotech van wereldformaat dat meespeelt in de internationale top. Niet voor niets viel de topman eerder al in de prijzen en namen ministers Hoekstra en Wiebes begin dit jaar nog een kijkje in het lab.

Vanzelf gaat het allerminst. Anderhalf jaar geleden al beschreven we deze transformatie, die in het zicht van de haven eind van de zomer nog een tegenvaller kreeg te verwerken. Het is exemplarisch voor de achtbaan waar zoveel biotech ondernemers zich in bevinden, waarbij een rit van 22 jaar zeker geen uitzondering is.

Wat HollandBIO betreft is het succesverhaal van Galapagos voortaan eerder regel dan uitzondering, maar ook dat zal niet vanzelf gaan. Zoals Onno het zelf zegt: ‘In beide steden [Leiden & Mechelen, red.] hadden we laboratoria. België heeft een goed klimaat voor biotechnologie. De Vlaamse overheid wil daar geld in investeren. En het beleid is consistent. Regeringen veranderen en regelmatig is er niet eens een regering, maar dat maakt voor het biotech beleid niets uit. In Nederland wordt het beleid bij elke nieuwe regering weer over de kop gegooid.’ Kortom, ook biotech beleid vergt engelengeduld. HollandBIO wil dat het niet 22 jaar hoeft te duren voordat nog een biotech van Nederlandse bodem mag uitgroeien tot wereldtop. Reden genoeg om onze schouders te zetten onder een ijzersterk innovatieklimaat!

Meer inzicht in beleidsstrategieën voor medicijnprijzen

Hoeveel mag een geneesmiddel kosten? Wat is een maatschappelijk aanvaardbare prijs? Belemmeren hoge prijzen de toegankelijkheid? Lastige vragen, waar geen klip-en-klaar antwoord op bestaat en waar meerdere partijen mee worstelen.

Om overheden een handvat te bieden voor het opstellen van een effectief geneesmiddelenbeleid op het gebied van vergoeding en toegankelijkheid van geneesmiddelen, heeft de WHO een herziening van de richtlijn “WHO guideline on country pharmaceutical pricing policies” gepubliceerd. Een helder overzicht, waarin de WHO acht principes voor het formuleren en opstellen van beleid rondom prijzen van geneesmiddelen en tien voorbeelden van beleidsstrategieën – zoals o.a. value-based pricing, kostprijsplusmodel, gebruik van referentieprijzen en gezamenlijke inkoop – de revue laat passeren. Hoewel de richtlijn primair bedoeld is voor beleidsmakers, is hij zeker ook het lezen waard voor liefhebbers van het geneesmiddelenbeleid.

Om ook patiëntvertegenwoordigers te informeren over dit soort ingewikkelde vraagstukken organiseerde patiëntenorganisatie Inspire2Live recent een webinar over de betaalbaarheid en toegankelijkheid van innovatieve behandelingen, gegeven door Prof. Carin Uyl-de Groot. Hoewel hier slechts één mogelijke beleidsstrategie in detail belicht werd, namelijk het kostprijsplusmodel, juicht HollandBIO het toe dat ook patiëntvertegenwoordigers zich bekwamen in deze materie en zo een goed geïnformeerde professionele partner kunnen zijn in de ontwikkeling en beoordeling van nieuwe geneesmiddelen. Procesinnovatie als oplossing voor de medicijnen van morgen

Een position paper naar ons hart, de nieuwe publicatie van TNO. We zijn steeds beter in staat om patiënten van complexe en gepersonaliseerde medicatie te voorzien. Maar het proces van geneesmiddelontwikkeling is duur, tijdrovend en risicovol. Dat kan sneller en beter: door vol in te zetten op technologische innovaties die het proces versnellen, zoals bijvoorbeeld de Accelerator Mass Spectrometer. TNO pleit voor een gezamenlijke en gecoördineerde aanpak om het proces van medicijnontwikkeling substantieel te verbeteren. We moeten op zoek naar de win-win voor alle betrokken partijen in het systeem, zodat we oplossingen realiseren waarbij gezondheidswinst, innovatie én betaalbaarheid hand in hand gaan, een kolfje naar HollandBIO’s hand.

FDA grants Glycostem’s oNKord® Orphan Drug Designation for Multiple Myeloma

Glycostem announces it has received the FDA’s Orphan Drug Designation (ODD) for treatment of Multiple Myeloma (MM) patients with its investigational product oNKord®. The designation will provide Glycostem with certain incentives, like eligibility for 7 years of market exclusivity and clear FDA guidance on specific aspects of development for rare diseases. These pave an accelerated path towards market access and treatment of patients suffering from this relatively rare form of cancer.

oNKord®is Glycostem’s first-generation off-the-shelf Natural Killer (NK) cellular immunotherapy product. Over the coming months, AML patients will receive this form of treatment as part of a phase I-IIa (pivotal) trial in AML. A phase II trial for MM patients isexpected to start in 2021. This makes Glycostem one of the frontrunners in this promising field of cellular immunotherapy.

“Since 2012 we have been pioneers in the field of developing and manufacturing off-the-shelf Natural Killer cell therapy products for cancer treatment. MM is the second most common blood cancer, accounting for 15% of blood cancers, and 2% of all cancers. In the US alone it affects more than 130,000 patients; approximately 32,000 Americans are diagnosed with MM each year. MM occurs in infection-fighting plasma cells (a type of white blood cell) found in the bone marrow. These cancerous cells multiply, produce an abnormal protein and push out other healthy blood cells from the bone marrow.

Promotie: ook minder HPV-infecties bij ongevaccineerde vrouwen

HPV-infecties komen sinds de start van het HPV-vaccinatieprogramma behalve bij gevaccineerde vrouwen ook minder voor bij ongevaccineerde vrouwen en heteroseksuele mannen. Er is sprake van groepsbescherming. Dit blijkt uit het promotieonderzoek van Petra Woestenberg naar de effecten van de HPV-vaccinatie voor meisjes die in 2009 binnen het Rijksvaccinatieprogramma is gestart.

Meisjes krijgen in het jaar dat zij 13 worden een uitnodiging voor HPV-vaccinatie. Daarmee zijn zij tenminste tot 8 jaar na vaccinatie goed beschermd tegen de 2 typen HPV (16 en 18). Deze typen veroorzaken 70% van alle gevallen van baarmoederhalskanker. Uit het onderzoek van Woestenberg blijkt ook dat het vaccin bescherming biedt tegen een aantal andere typen HPV die ook kanker kunnen veroorzaken. Dit wordt kruisbescherming genoemd. Verder lijkt het erop dat het bestaande vaccin deels bescherming biedt tegen anogenitale wratten.

Uit de gegevens tot 8 jaar na de start van het vaccinatieprogramma blijkt dat er sprake van is groepsbescherming: HPV-infecties door HPV16 en 18 en de typen waarvoor kruisbescherming is aangetoond, komen minder voor bij heteroseksuele mannen. Bij ongevaccineerde vrouwen is bescherming aangetoond tegen de HPV-typen in het vaccin.

Voor haar proefschrift baseerde Woestenberg zich op data uit de PASSYON-studie. In deze studie, die tegelijk startte met het HPV-vaccinatieprogramma, wordt elke 2 jaar een dwarsdoorsnedeonderzoek gedaan onder jongeren van 16-24 jaar die centra seksuele gezondheid bezoeken. Zij werden o.a. op verschillende typen HPV getest. Op basis van deze gegevens is het directe (individuele) effect van de vaccinatie gemeten en in hoeverre het voorkomen van HPV-infecties in de bevolking is veranderd.

Mymetics Starts Preclinical Studies with Baylor College of Medicine for Virosome-based Covid-19 Vaccine

Leestip: Rolt het coronavaccin straks in Oss van de band?

Waar een klein land groot in zou kunnen zijn. Zo kan er bij BioConnection in Oss in het gunstige geval 200 miljoen dosissen per jaar verwerkt kunnen worden voor een vaccin of medicijn tegen het coronavirus. BioConnection werd vijftien jaar geleden opgericht door Organon, met Mibiton en de Brabantse Ontwikkelings Maatschappij als medeaandeelhouders, en heeft anderhalf jaar geleden een nieuwe productielijn van wel 25 meter lang laten aanrukken. Daarmee kan deze producent ontwikkelaars bijstaan in de grote taak die ons nog te wachten staat: het produceren van een medicijn of vaccin tegen het coronavirus. Stappenplan implementatie coronavaccins bekend: nu nog afstemmen

Met een inzichtelijke Kamerbrief en Routekaart Vaccinatie speelt de overheid open kaart over haar voorbereidingen op een succesvolle implementatie van veilige en effectieve coronavaccins. Ook hier vormt een parallelle aanpak, zoals we eerder zagen bij het ontwikkelproces van de vaccins, de sleutel tot een spoedige start. Nu “alleen nog” de werelden van beleid en productie samenbrengen. Tijdig afstemmen is geen overbodige luxe, gezien het enorme logistieke huzarenstuk rond de productie en de verdeling van vaccins. Laten we daarom de vaart erin houden met -hoe kan het ook anders- een parallel afstemmingtraject tussen alle betrokken stakeholders.
