Uitgebreide DNA-test voor iedere kankerpatiënt ideale wegbereider voor implementatie van technologische innovaties

Mensen lijken in veel opzichten op elkaar en tegelijkertijd zijn we allemaal uniek. Je gezondheid is van heel veel factoren afhankelijk: DNA, leefstijl, waar je woont, wat voor werk je doet, wat je eet, maar ziekte is ook vaak domme pech. Wanneer je ziek bent, wil je maar één ding en dat is beter worden. Maar wat werkt voor jou, hoeft niet te werken voor een ander. Gelukkig zijn we door technologische innovatie steeds beter in staat om te bepalen wat voor een individueel persoon de beste behandeling is.

Een prachtig voorbeeld van zo’n technologische innovatie is de analyse van het complete tumor-DNA, met behulp van Whole Genome Sequencing (WGS). Deze test kan helpen bij het selecteren van de juiste persoonlijke behandeling of om te achterhalen waar de tumor is ontstaan. De data kan ook helpen om nieuwe toekomstige behandelingen te ontwikkelen. Het Antonie van Leeuwenhoek zet samen met Hartwig Medical Foundation sinds begin dit jaar deze uitgebreide DNA-test in voor reguliere diagnostiek. Afgelopen week sprak de Tweede Kamer over wat er nodig is om een WGS-test zo snel mogelijk voor alle kankerpatiënten in Nederland beschikbaar te maken. Dit naar aanleiding van de initiatiefnota over de urgentie van invoering uitgebreid persoonlijk profiel. Onze inbreng, evenals die van andere partijen, vind je hier.

De belangrijkste boodschap wat HollandBIO betreft? Een persoonlijk DNA-profiel voor iedere kankerpatiënt in Nederland is een uitstekende wegbereider, maar de lat moet omhoog. De HollandBIO-achterban bulkt van de ambitie en potentie om te zorgen dat iedere patiënt de juiste zorg op het juiste moment krijgt én de ontwikkeling van nieuwe behandelingen wordt bespoedigd. Van start-ups tot scale-ups en multinationals, van samenwerkingsverbanden tot stichtingen: ze ontwikkelen óf gebruiken slimme meet- en analysemethoden als (whole genome) sequencing, 3D-celkweek, organoïden, organs-on-a-chip, single cell sequencing, imaging, bioinformatica en artificial intelligence. De enorme potentie van deze methoden werd afgelopen week nog geïllustreerd in het FD, waarin kinderoncoloog Van den Heuvel-Eibrink uitlegt hoe organoïden in de nabije toekomst kunnen helpen bij het selecteren van de beste behandeling. Wat HollandBIO betreft verdienen deze nieuwe technologieën en toepassingen een duurzame en heldere route van lab naar praktijk. Daar zet HollandBIO zich voor in.

InteRNA Technologies Announces Dosing of First Patient in First-in-Human Trial of microRNA Drug Candidate INT-1B3 in Patients with Advanced Solid Tumors

InteRNA Technologies announced today that the first patient has been dosed in the first cohort of its first-in-human Phase I study with the Company’s lead microRNA candidate, INT-1B3. The trial will evaluate safety and initial signs of efficacy of INT-1B3, a microRNA-mimic of the endogenous tumor suppressor miR-193a-3p formulated in next-generation lipid nanoparticles, in patients with advanced solid tumors. INT-1B3 represents a promising novel therapeutic approach that could address multiple hallmarks of cancer simultaneously by directly targeting tumor cells and the tumor microenvironment by specific modulation of multiple relevant signaling pathway components triggering a long-term T cell-mediated immune response against the tumor.

“The enrollment of the first patient in this trial, especially during the ongoing COVID-19 pandemic, is a significant milestone for InteRNA,” said Dr. Roel Schaapveld, CEO of InteRNA Technologies. “microRNAs represent a new class of therapeutic agents that have the potential to change the treatment paradigm in immune-oncology, delivering a combination approach as a single regimen. This two-part, open-label, multiple ascending dose Phase I/Ib trial (NCT04675996) will evaluate the safety, pharmacokinetics, pharmacodynamics, and preliminary efficacy of INT-1B3. The dose escalation part of the study (part 1) will be conducted in trial sites located in the Netherlands and Belgium and will enroll approximately 30 patients with advanced solid tumors. The dose expansion part of the trial (part 2) will be conducted in multiple clinical study centers located in several European countries as well as in the United States and will enroll up to 50 patients with hepatocellular carcinoma or triple negative breast cancer. All patients will receive INT-1B3 via infusions twice per week in 21-day cycles. Topline results from the dose escalation part of the study are expected by the end of 2021.

European Commission grants Vico Therapeutics Orphan Drug Designation for VO659, an Investigational Therapy for Spinocerebellar Ataxia

Vico Therapeutics,a Leiden, the Netherlands, based biotech company focusing on the development of RNA modulating therapies for rare neurological disorders, announced that the European Commission (EC) has granted orphan drug designation for VO659, Vico’s investigational antisense oligonucleotide therapy for the treatment of Spinocerebellar Ataxia (SCA). The orphan designation was based on a positive opinion from the European Medicines Agency (EMA) Committee for Orphan Medicinal Products (COMP). Vico previously received orphan drug designation for VO659 in Huntington Disease.

“Spinocerebellar Ataxias make up a great part of the so called polyglutamine disorders, debilitating and progressive diseases, leading to significant impairment of mobility, speech, and multiple other daily activities of patients suffering from this condition. So far, there is no treatment which can halt the progression of these diseases,” said Rupert Sandbrink M.D. Ph.D., Chief Medical Officer at Vico.

“Our investigational RNA modulating therapy is designed to lower the mutant protein levels causing these neurodegenerative diseases. We’re pleased to receive orphan drug designation, which recognizes both the unmet medical need for patients living with spinocerebellar ataxias, and the potential of our approach.”

Qualified for orphan drug designation are drugs and biologics intended for the safe and effective treatment, diagnosis or prevention of rare diseases or conditions that impact fewer than 5 in 10,000 patients in the European Union. Orphan drug designation gives developing companies certain benefits, including clinical protocol assistance, reduced regulatory fees, research grants and 10 years of market exclusivity following regulatory approval.

Vaccins produceren is geen pannenkoeken bakken

De afgelopen maanden lukte wat niemand voor mogelijk hield: het ontwikkelen, registreren en inzetten van niet één, maar meerdere vaccins tegen het coronavirus. Een prestatie van de wereldwijde biotech sector waar we bij HollandBIO U tegen zeggen. Zaak is nu om de vaccinproductie zo snel mogelijk op te schalen, zodat iedereen, waar dan ook ter wereld, binnen zo kort mogelijke tijd gevaccineerd kan worden. En ook dat is een uitdaging van jewelste, waarvoor steeds meer vaccinontwikkelaars en -producenten middels samenwerkingsverbanden de handen ineen slaan. Hoewel het initiatiefwetsvoorstel over dwanglicenties van Tweede Kamerleden Ellemeet en Ploumen anders doet vermoeden, is een vaccin produceren vele malen complexer dan een het bakken van een pannenkoek – en zelfs bij het bakken van pannenkoeken, mislukt de eerste steevast. Het produceren van een vaccin is het bereiden van een sterrendiner: noch de toegang tot de ingrediënten, noch de toegang tot het recept geeft garantie op een restaurantwaardig resultaat. Laat staan dat je in afwezigheid van keukencapaciteit binnen afzienbare tijd de hele wereld op een hoogstaande culinaire beleving kan trakteren.

De wereld verlossen van een pandemisch virus door het zo snel mogelijk ontwikkelen en produceren van veilige en effectieve vaccins, is bij uitstek een internationale aangelegenheid. Bedrijven onderzoeken en ontwikkelen hun vaccin in een R&D-centrum, al dan niet in samenwerking met universiteiten. Dat is een heel andere tak van sport dan de fase waarin een aantal fabrikanten nu (gelukkig!) verkeren: grootschalige productie. Bedrijven zetten alles op alles om ook deze klus zo snel en goed mogelijk te klaren. Waar de productie plaatsvindt, of ze dit zelf doen, uitbesteden, of beiden, is van ondergeschikt belang. De diverse samenwerkingen die farmaceuten aangaan laten dit zien: zo gaan Sanofi en Novartis helpen bij de productie van het vaccin van Pfizer/BioNTech, terwijl GSK en Bayer dat van plan zijn voor het CureVac-vaccin. Contract Manufacturing Organizations overal ter wereld draaien overuren. In De Telegraaf van 6 februari gaat HollandBIO’s Annemiek Verkamman verder in op de vraag waarom niet alle stappen in het proces in Nederland, of zelfs Europa, plaatsvinden.

Het in korte tijd opschalen van de productie naar ettelijke miljarden vaccins is geen sinecure en gaat dan ook zo nu en dan gepaard met tegenslag. De hele wereld kijkt mee, en iedereen wil liever vandaag dan morgen een prik in zijn arm. Geen wonder dat de productie nauwgezet gevolgd wordt en gepaard gaat met de nodige verhitte discussies en debatten. Verschillende Europarlementariërs, Tweede Kamerleden, beleidsmakers en media voeren de druk op om vaccinontwikkelaars aan te sporen maximaal te produceren en te leveren, en werkelijk alle beschikbare productiecapaciteit te benutten. En terecht ook.

Een suggestie waar onder andere Tweede Kamerleden Ellemeet (GroenLinks) en Ploumen (PvdA) mee trachten te scoren, is door de afgifte van dwanglicenties te vergemakkelijken, in combinatie met het doorstrepen van data-exclusiviteit. Dreigen met, of het verlenen van dwanglicenties zet echter geen zoden aan de dijk. Octrooien zijn geen kant-en-klare gebruiksaanwijzing, noch geven zij, net als de ingrediënten en receptuur van een sterrendiner, garantie op een succesvol resultaat. Octrooien zijn geenszins de beperkende factor bij het opschalen van productie, zoals geschetst door Mark Jolink, octrooigemachtigde bij octrooibureau EP&C, in een recent item bij BNR. Het is eveneens de boodschap van het merendeel van de organisaties, waaronder HollandBIO, tijdens de consultatie over een initiatiefwet van eerder genoemde Tweede Kamerleden, die beoogt het afgeven van dwanglicenties te versoepelen in Nederland.

HollandBIO is ervan overtuigd dat niet eenzijdig ingrijpen, maar juist samenwerking de sleutel is tot succes. Door als overheden, vaccinontwikkelaars en andere partijen samen op te trekken zoals we nu al in toenemende mate zien, hebben we alle ingrediënten, receptuur, expertise én keukengerei in huis om wél het beoogde resultaat te boeken.

De coronavrije samenleving heeft een deltaplan infectieziekten nodig

Ook in de winter komt onze favoriete Zomergast Jaap Goudsmit met een pleidooi waar HollandBIO warm van wordt. In Buitenhof pleit hij opnieuw voor het opstellen van een Deltaplan Infectieziekten, al heeft het pleidooi na de vorige keer allerminst aan urgentie ingeboet. De huidige coronacrisis laat goed zien dat investeren in een goed uitgewerkte strategie rond preventie van infectieziekten de sleutel is om deze uitdagingen het hoofd te bieden.

Niet alleen voor corona, ook voor andere toekomstige gezondheidscrises is preventie de sleutel tot succes. Laten we lessen trekken uit de huidige pandemie, en zorgen dat we beter ingespeeld zijn op toekomstige uitbraken van infectieziekten. Ook wij blijven het zeggen: de biotechindustrie kan een cruciale bijdrage leveren met de ontwikkeling van tests, vaccins en behandelingen voor infectieziekten.

Catapult Therapeutics receives FDA Clearance of Investigational New Drug (IND) Application for CAP-100, an innovative first-in-class humanized anti-CCR7 antibody

Catapult Therapeutics, a biopharmaceutical company developing novel cancer treatments, announced the U.S. Food and Drug Administration (FDA) has cleared the Investigational New Drug (IND) application for its lead product candidate CAP-100, an innovative first-in-class humanized anti-CCR7 antibody for treatment of hematological malignancies.

Catapult Therapeutics is now poised to initiate a Phase 1 clinical trial in patients with relapsed or refractory CLL (chronic lymphocytic leukemia). Expected to begin in the second quarter of 2021, the clinical trial is designed to evaluate the safety, tolerability, pharmacokinetics, pharmacodynamics, and preliminary anti-tumor activity of CAP-100. The study will be performed at Dana-Farber Cancer Institute in Boston, where startup activities are currently underway, and two other leading clinical sites in the USA.

Minder dierproeven door inzet van technologische innovaties

Het blijft een pijnlijke paradox: om toekomstig leed bij mens en dier te voorkomen of tegen te gaan, zijn dierproeven nog steeds alledaagse praktijk. In 2019 werden er voor toxicologisch onderzoek 64.000 dierproeven uitgevoerd in Nederland, zo bleek uit de nieuwste cijfers van de Volkskrant afgelopen weekend. Eerder gingen in diezelfde krant wetenschappers met elkaar de discussie aan over nut en noodzaak van proefdiervrij onderzoek voor de ontwikkeling van nieuwe geneesmiddelen en vaccins. Toch ziet HollandBIO proefdiervrij onderzoek stap voor stap aan terrein winnen.

Bovendien kan proefdiervrij onderzoek een logisch voortvloeisel zijn van de ambitie om nieuwe geneesmiddelen en vaccins sneller en beter te ontwikkelen. HollandBIO denkt dat door de inzet van technologische innovaties steeds meer dierproeven, bijvoorbeeld voor toxicologisch onderzoek, vervangen kunnen worden door alternatieve proefdiervrije methoden. Zo kondigden Toxys, de Universiteit Leiden en het LUMC afgelopen week de ontwikkeling en verdere commercialisatie van Toxprofiler aan, een proefdiervrij alternatief voor chemische veiligheidstesten. Een mooi voorbeeld van hoe technologische innovaties meer proefdiervrij onderzoek voortbrengen.

Nieuw vaccin tegen Rift Valley fever veilig en effectief in drachtige schapen

Onderzoek uitgevoerd door Wageningen Bioveterinary Research (WBVR), in samenwerking met BunyaVax en Ceva Animal Health, laat zien dat een nieuw levend-verzwakt kandidaat-vaccin tegen Rift Valley fever virus (RVFV) veilig en effectief is in drachtige schapen. Eerder ontwikkelde levend-verzwakte vaccins bieden de ooi bescherming maar bleken gevaarlijk voor de ongeboren vrucht.

RVF is een door muggen overgedragen virus dat herkauwers en de mens kan infecteren. Vooral schapen zijn gevoelig voor het virus. De gevolgen van een RVFV-besmetting zijn vooral ernstig in pasgeboren lammeren, waarvan de meeste dieren de infectie niet overleven. Bij drachtige schapen resulteert RVF-infectie in abortus. Infecties bij de mens resulteren doorgaans in griepachtige symptomen, een klein percentage ontwikkelt ernstige ziekte die fataal kan aflopen. Het virus komt momenteel alleen voor op het Afrikaanse continent en het Arabisch schiereiland. De muggen die het virus kunnen overdragen komen wereldwijd voor, waaronder in Nederland. Globalisering, klimaatverandering en toenemende populaties van dieren en mensen vergroten het risico op toekomstige uitbraken van RVFV wereldwijd.

Een levend-verzwakt RVF als vaccin

Er bestaan geïnactiveerde en levend-verzwakte vaccins. Geïnactiveerde RVF-vaccins zijn veilig voor dieren, inclusief voor de ongeboren vrucht. De optimale werkzaamheid van deze vaccins hangt echter af van meerdere vaccinaties en jaarlijkse hervaccinatie. Levend-verzwakte vaccins zijn over het algemeen zeer effectief na een enkele vaccinatie, maar de tot nu toe beschikbare levend-verzwakte vaccins tegen RVF kunnen worden overgedragen op de foetus, wat kan leiden tot doodgeboorte, aangeboren afwijkingen en abortus. In tegenstelling tot deze vaccins bleek het nieuwe RVF-vaccin volledig veilig voor drachtige dieren, zelfs na toediening van een zeer hoge dosis. De huidige studie laat zien dat het levend-verzwakte vaccin volledige bescherming geeft na een enkelvoudige vaccinatie.

RVF-vaccin voor de mens

De technologie die gebruikt is voor de ontwikkeling van het vaccin voor dieren wordt ook toegepast voor de ontwikkeling van een RVF-vaccin voor gebruik bij mensen. Dit kandidaat-vaccin wordt momenteel ontwikkeld in het LARISSA-project dat gefinancierd wordt door de Coalition for Epidemic Preparedness Innovations. Eind 2021 zal dit vaccin worden getest op veiligheid bij mensen.

NICE recommends Jyseleca® ▼(filgotinib) on NHS in landmark decision for rheumatoid arthritis

Galapagos NV (Euronext & Nasdaq: GLPG) welcomed the news that the National Institute for Health and Care Excellence (NICE) has issued a final appraisal determination (FAD) recommending the use of the daily oral pill, JYSELECA®▼ (filgotinib) on the National Health Service (NHS) in England for the treatment of eligible adult patients with moderate to severe active rheumatoid arthritis (RA).1 It is the first time in the UK that an advanced therapy has been recommended in people with moderate RA, offering thousands more the potential to achieve remission earlier – potentially slowing the irreversible damage and life-limiting symptoms RA can cause.2 RA is a degenerative auto-immune disease that can cause life-threatening complications.4 The sooner treatment begins, the better the chance of slowing disease progression.2 With thousands of people potentially eligible, the recommendation could help improve many lives as well as lessen the significant societal burden RA has in England.5,6

 “We are delighted with the NICE recommendation for Jyseleca today. For patients with moderate to severe RA in England this decision represents a significant new opportunity and especially for those with moderate symptoms who can now receive an advanced treatment earlier,” said Onno van de Stolpe, Galapagos CEO.

Filgotinib is a once daily oral pill that can be given on its own (as a monotherapy) or used alongside another common RA medicine, called methotrexate.7 Eligible patients with moderate or severe RA will have responded inadequately to intensive therapy with 2 or more conventional disease-modifying antirheumatic drugs (DMARDs).1 Eligible patients with severe disease will also have wider access to filgotinib in line with criteria defined by NICE. Filgotinib is an advanced therapy which, in RA, is a term used to describe biologic DMARDs and targeted synthetic DMARDs.1

More than 400,000 people in the UK live with RA (around 380,000 in England), and it is recognised as a condition that can cause debilitating physical pain, affect mental health and require chronic care.2 Studies have shown that RA shortens life expectancy, with some estimates putting this at around 10 years.8 Nearly 50% of patients diagnosed with RA suffer from mental health issues with 1 in 6 people having a major depressive disorder.6,9 RA is also a significant burden on the UK economy. Around a third of people diagnosed with RA stop work within two years of diagnosis10 and the combined cost of workdays lost due to osteoarthritis and RA in the UK was estimated at £2.58 billion in 2017 – estimated to rise to £3.43 billion by 2030.5

NICE guidance covers England. Wales and Northern Ireland are expected to follow the guidance with timelines for implementation currently under consideration. Filgotinib will be reviewed separately by the Scottish Medicines Consortium for use on the NHS in Scotland.

Under a new arrangement between Gilead and Galapagos, announced in December 2020, Galapagos will assume sole responsibility for filgotinib in Europe, including the UK. Through a phased transition the majority of activities supporting filgotinib in Europe are expected to be assumed by Galapagos by the end of 2021.

First Patient Treated in Phase II Combination Trial of ISA Pharmaceuticals’ Lead Immunotherapy ISA101b with Keytruda®

ISA Pharmaceuticals B.V. today announced that the first patient has been dosed with its lead product ISA101b in a Phase II trial conducted and sponsored by UPMC Hillman Cancer Center, the University of Pittsburgh, PA, and supported by Merck Sharp & Dohme Corp and ISA Pharmaceuticals B.V.

The study in previously untreated patients with intermediate-risk head and neck cancer associated with human papilloma virus subtype 16 (HPV-16) will evaluate the efficacy of ISA101b and pembrolizumab (Keytruda®) in combination with cisplatin and radiotherapy. The principal investigator is Robert L. Ferris MD, PhD, Director of UPMC Hillman Cancer Center in Pittsburgh and Associate Vice Chancellor for Cancer Research at the University of Pittsburgh School of Medicine. Dr. Ferris is a paid member of the US Scientific Advisory Board for the Merck Sharp & Dohme Corp. This study was supported in part by a research grant from the Investigator-Initiated Studies Program of Merck Sharp & Dohme Corp.

The investigator-initiated, single-arm, open-label ‘Phase II Study Evaluating HPV-16 vaccination (ISA101b) and Pembrolizumab plus Cisplatin Chemoradiotherapy for Intermediate Risk HPV-16 associated Head and Neck Squamous Cell Carcinoma’ aims to enroll 50 patients with end of enrollment expected in late 2022.

The primary endpoint of the trial is 2-year progression-free survival (PFS) in patients with newly diagnosed, local-regionally advanced, intermediate risk HPV-associated head and neck cancer.

“For head and neck cancer patients at risk for tumor recurrence, chemo- and radiation therapy are being combined with PD-1 immune checkpoint inhibitors,” said Dr. Ferris. “We seek to leverage the effect of immunotherapy to enhance T cell specific immune responses against HPV, to provide for a more profound and durable anti-tumor effect. To explore this, we have chosen the synthetic long peptide (SLP) therapeutic HPV immunotherapy ISA101b. ISA Pharmaceuticals has demonstrated that HPV, as the predominant cause of this type of cancer, harbours powerful antigens against which virus-specific T cells will mount an attack on the cancer, and we will attempt to enhance this benefit using anti-PD1 pembrolizimab.”

“Currently immunotherapies combined with additional treatments are emerging as the most promising approach in oncology,” said Prof. Cornelis Melief, Chief Scientific Officer of ISA Pharmaceuticals. “Consistent with this, we have demonstrated in multiple trials that ISA101b is highly synergistic with various complementary cancer treatments. Our product has shown very promising results in Phase II combination trials with standard chemotherapy1 and the checkpoint inhibitor nivolumab2. This new trial will provide valuable insights into the effect of immunomodulation in the early stage adjuvant setting, in addition to our ongoing randomized Phase II ISA101b trial with cemiplimab in late stage oropharyngeal cancer.
